After vaccination, patient experienced the following side effects: angioedema, blister, dyspnoea, hypoaesthesia, pain"Approximately 1 wk following injection: large hives 4" to 6" (blister like) on both legs from knee to hip for about 1 week- shooting pains in hands-followed by numbness in fingers- now in finger tips and pain in Achilles tendons (still hurts but not so acute) still injured and shortness of breath.". NonePenicillin . Patient recovered.

After vaccination, patient experienced the following side effects: chest pain, dyspepsia, dyspnoea, eructation, urticaria45 minutes after injection - developed bilat. chest pain and around to back. Short of breath. Took Benadryl 50mg PO and felt better in 5-10 min. Also had a lot of indigestion and belching.. NoneAllergies: MorphineNone . During the same period patient was treated with HCTZ 25mg qd; Lipitor 10mg one qd.. Patient recovered.

INFLUENZA (FLUVIRIN) side effect was reported . Female patient, 65.0 years of age, was vaccinated with INFLUENZA (FLUVIRIN). After vaccination, patient experienced the following side effects: dyspnoea, electromyogram abnormal, eye pain, eye pain, headache"Acute onset of excruciating right shoulder and upper arm pain following flu virus immunization. Pain intensity very high (9+/10) and maximal at onset. Pain is sharp and throbbing and constant since injection. Deep aching and muscle weakness and atrophy in upper right arm. Also, sharp/shooting pain affecting eyes and head. Numbness and muscle weakness continuing chronic right upper arm and shoulder pain, significant muscle weakness continuing with physicians for diagnostic work up. 4/3/09-records received-office visit 1/21/09-C/O 3 months of right upper arm lateral aspect being painful with decreased strength. 4/16/09 neurology consult received for DOS 12/16/08-C/O headaches with black spots in front of eyes. Anxiety worsenes tension type headaches. Impression: chronic daily headaches both migraine and tension type and analgesic rebound headache. Daytime somnolence snoring and gasping respiration and weight gain. Lancinating headaches. 2/2/09-C/O right upper arm pain since receiving flu shot.". No illness"Arrhythmia, migraines, hypothyroidism, hypertension, degenerative dise disease. 4/3/09-records received-PMH:HTN, arrhythmia with PVCs, hypothyroidism, HRT, GERD, chronic low back pain with sciatica, osteoarthritis. 4/16/09-records received-PMH migraine headaches and daily headaches for 20 years.""Neuropathy, abnormal EMG and nerve conduction study 2/09/2009. Physical therapy, blood labs, (CBC), (ANA) ordered. 4/3/09-records received-EMG and nerve conduction study abnormal evidence of minimal right sided median neuropathy loss of se" . INFLUENZA (FLUVIRIN) side effect was reported on 04/27/2009. Male patient, 40.0 years of age, was vaccinated with INFLUENZA (FLUVIRIN). After vaccination, patient experienced the following side effects: acute disseminated encephalomyelitis, asthenia, csf protein normal, csf white blood cell count negative, demyelination"After I received the flu shot I developed sever numbness on my extremities witihn 2-3 weeks. After that I developed severe weakness, loss of motor power mostly on the right hand and leg, tachycardia, sever muscle twitches, contracture on right lower extremity, difficulty breathing because of tachycardia. I was diagnosed as Acute dissemintaed encephalomyelitis (ADEM) and my Brain MRI showed diffuse demyelinization of the brain. I recovered mostly. I report this after I finished my follow up with neurology departmnts to make sure it is not a presentation of MS. I received 2 courses of steroids meanwhile that was not helpful. I still have numbness on my right leg and foot and muscle twitches. 5/28/09 Medical records received DOS 3/11/08 to 2/2/09. FINAL DIAGNOSIS: Acute disseminated encephalomyelitis, cervical stenosis. Post vaccination presented with numbness of hands feet face. Generalized weakness and fatigue. Shortness of breath, tachycardia, PVCs. Multifocal muscle twitching. 6/8/09 Hospital record (Multiple Sclerosis Center) received DOS 4/1/09. FINAL DIAGNOSIS: Acute disseminated encephalomyelitis, cervical stenosis. Follow up exam received. No new codes other than patient reports uveitis treated with prednisone eye drops in June 2008 and pericarditis treated with ibuprofen in July 2008.\ksk". Uveitis, Mycarditis."Lumbar puncture 2 times within 9 months interval was negative for possible MS panel. Four more Brain MRIs were negative for new lesions. EMG and NCV were Normal. Visual evoked potential was normal. Lumbar puncture did not show any elevated . During the same period patient was treated with "omeprazole , Simvastatin. 5/28/09 Medical records received DOS 3/11/08 to 2/2/09. omeprazole, simvastin, clonazepam, acetaminophen, corticosteroids. After vaccination, patient experienced the following side effects: back pain, blood product transfusion, csf cell count normal, cryptococcus antigen, diabetes mellitus"Flu like symptoms for two to three weeks. Numbness in feet - leg cramps - back pain in spinal area - problem walking (paralysis). Hospitalization - tests and treatment for Guillain-Barre (GBS) and acute rehabilitation - 30 day hospitalization.4/29/09-records received-for DOS 1/20-2/3/09-DC DX: Ascending motor sensory weakness secondary to Guillain-Barre Syndrome following recent viral respiratory tract illness 2-3 weeks prior to admission. Status post IVIG. Uncontrolled diabetes mellitus secondary to noncompliance with diet and medication. Hypertensive emergency improved. Ambulatory dysfunction secondary to gait instability. Hyponatremia. Hypokalemia. Presented with increasing gait instability and lower extremity paresthesia. Increased thirst, increased urination and fatigue. Numbness progressed to upper extremities, right facial weakness, difficulty swallowing and some dyspnea". INFLUENZA (FLUVIRIN) side effect was reported on 05/26/2009. Male patient, 14.0 years of age, was vaccinated with INFLUENZA (FLUVIRIN). After vaccination, patient experienced the following side effects: agitation, alanine aminotransferase increased, areflexia, aspartate aminotransferase increased, bacterial culture positive"Several weeks (end of November) after getting flu vaccine, client began experiencing weakness in legs with numbness in feet and toes. He began walking with a cane and 2 days later he was in a wheelchair. Family took client to the ER when he couldn't swallow. Admitted to hospital on December 3rd, 2007, and diagnosed with Guillian-Barre Syndrome. He was on life support X 17 days and in ICU X 1 month. Transferred to rehab center. 5/27/09-records submitted with report-Presented on 12/2/08 with 10 day history of increasing weakness and numbness of feet which made an ascending pattern up entire body. 4 days prior began with feelings of breathlessness and day before presenting to ED C/O trouble with swallowing and muffled speech. Elevated BP on admission 148/99 and subsequent transient increase in BP. Treated with IVIG. Mechanical ventilation required with intubation and subsequent extubation. NG feedings. DX: Guillain-Barre syndrome. One month hospitalization. Office visit follow up 1/23/09 residual weakness on both sides of face, CN VII focally affected R>L. 6/26/09 Hospital records and DC summary recieved. DOS 12/02/07 to 01/04/08. Assessment: Ascending paralysis, probable Guillain-Barre. Patient presented with ascending numbness and weakness. Nausea and vomitjng. Numbness in feet. Difficulty swallowing liquids. Unable to walk or sit up on his own. DTR's absent. Stregnth diminished upper and lower extremities. Fever, headache. Sedation/Analgesia dependence. Agitation with respiratory distress. Hypertension.
