Evening of 10/23/08, had increased sinus congestion, body aches, fever/chills and unable to sleep. 10/24 AM, saw Dr. for viral infection and sinus infection. Given Zithromax. Relieved of fever & chills.". NoneAllergic to cats and dustNone . During the same period patient was treated with None. Patient recovered.

In morning noticed both arms red (from top of hands up to elbows). DX by Dr on 1/08/09 in office with Lymphangectasia and referred to Immunologist. After vaccination, patient experienced the following side effects: pruritus, pyrexia, urticaria, , "Parent reports adolescent having hives on hands,knees and feet that began approximately 2-3 days past receiving vaccines on 1/22/09. Also reports fever for one day when hives began. Using topical benadryl cream with no relief. Hives and c/o itching remain. Hives seen by RN in clinic today. Referred to primary physician.". After vaccination, patient experienced the following side effects: anorexia, aphasia, autism spectrum disorder, dental caries, dermatitis diaper"On 10.14.04 - Patient received FLU, DTAP, COMVAX, MMR & PNEUM. conj after this did not talk, quiet, sick, tired, fever (HOT) 3-4 d. ibu. given did not eat with in 2 wks, eyes rolling (thought nl after vacc). Still does not eat well. Mom thought was regressing & mad abt being held down. In bed 3d. Mom lived with guilt for 10 mo, finally presented to Dr 9-7-05. Very severe delays/regression. Does not speak (did at 11 mo). Very restricted diet, refuses most foods. 5/8/09 Received vaccine & PCP medical records. FINAL DX: Autism spectrum disorder; mixed receptive-expressive language disorder; feeding difficulties; encopresis; constipation; dental caries Records reveal patient experienced runny nose s/p cold week prior to vaccination on 10/14/04. Developed social withdrawal & speech loss. 2/24/05 dx w/RLL pneumonia; 12/05 bilat AOM & diaper rash; 4/06 viral URI; 10/06 viral pharyngitis.". None - URI just doneNone PMH: fever & cold s/s; viral syndrome w/rash; recurrent bilateral AOM; large anterior fontanelle. INFLUENZA (FLUVIRIN) side effect was reported on 05/26/2009. Male patient, 14.0 years of age, was vaccinated with INFLUENZA (FLUVIRIN). After vaccination, patient experienced the following side effects: agitation, alanine aminotransferase increased, areflexia, aspartate aminotransferase increased, bacterial culture positive"Several weeks (end of November) after getting flu vaccine, client began experiencing weakness in legs with numbness in feet and toes. He began walking with a cane and 2 days later he was in a wheelchair. Family took client to the ER when he couldn't swallow. Admitted to hospital on December 3rd, 2007, and diagnosed with Guillian-Barre Syndrome. He was on life support X 17 days and in ICU X 1 month. Transferred to rehab center. 5/27/09-records submitted with report-Presented on 12/2/08 with 10 day history of increasing weakness and numbness of feet which made an ascending pattern up entire body. 4 days prior began with feelings of breathlessness and day before presenting to ED C/O trouble with swallowing and muffled speech. Elevated BP on admission 148/99 and subsequent transient increase in BP. Treated with IVIG. Mechanical ventilation required with intubation and subsequent extubation. NG feedings. DX: Guillain-Barre syndrome. One month hospitalization. Office visit follow up 1/23/09 residual weakness on both sides of face, CN VII focally affected R>L. 6/26/09 Hospital records and DC summary recieved. DOS 12/02/07 to 01/04/08. Assessment: Ascending paralysis, probable Guillain-Barre. Patient presented with ascending numbness and weakness. Nausea and vomitjng. Numbness in feet. Difficulty swallowing liquids. Unable to walk or sit up on his own. DTR's absent. Stregnth diminished upper and lower extremities. Fever, headache. Sedation/Analgesia dependence. Agitation with respiratory distress. Hypertension. IVIG administered. Transferred to rehabilitation facility. ICD-9 Codes: 357.0, 518.81, V46.11, 707.03, 292.0, 304.01, 112.0, 401.9, 075, 726.87, 041.5, 564.00, 110.5".
