After vaccination, patient experienced the following side effects: erythema, oedema peripheral, skin warm, , "3 1/2 X 5 inch raised, erythematous area to right deltoid warm to touch. Vaccine given 12-22-08, reaction began on 12-23-08 around 1:00 pm. Reaction evaluated 12/24/08 @ clinic. Instructed ct if redness, swelling increases she need to go to ER. Due to Holiday ct to RTC 12-29-08 for reevaluation. 12-29-08 ct rtc for re-evaluation; erythema at swelling minimal. Ct went to ER 12-24-08. Given SOLUMEDROL, BENADRYL, pain medication. After vaccination, patient experienced the following side effects: body temperature increased, erythema, rash, screaming, urticaria"Had temp 102.4 evening following vaccine - mother took him to after hours pediatric clinic who told her to watch him - saw his regular MD on Wednesday - only had small at that time on arm. Mother states told doctor about visit to after hours ped clinic but he did not really address that issue. Child started according to mom a high pitched scream day after receiving shots and has been having it off and on since. She states he starts screaming when he awakens from sleep. She states he stops when you pick him up and rock him. She states he started having a rash 12-11-08 - red bumps on his face, chest, stomach, and on back. She states in A.M. rash is really bad but then goes to just being red areas later on in the day. Informed mom to make an appt with her pediatrician for evaluation. She voiced understanding of instructions. Called mom back today at 2:45 PM. She states child is doing better, redness in face gone - but still has bumps on both arms and his stomach. After vaccination, patient experienced the following side effects: areflexia, blood product transfusion, csf test normal, choking, computerised tomogram normal"Influenza vaccines rec'd 10/18/07 and 11/20/07. 3 mos later developed Guillain-Barre syndrome/was hospitalizsed for acute lower extr. weakness. 5/19/09 hospital records received DOS 2/13/08 to 2/18/08. FINAL DIAGNOSIS: Acute lower limb weakness, possible Guillain-Barre Syndrome. Post vaccination: Choking episode, weakness and inability to bear weight on lower limbs. Fails to sit up or crawl. Otitis media. Decreased tone and power lower limbs. Biceps/triceps reflexes weak. Absent patellar reflexes. Weak ankle reflexes. Tachypnea, limp 3 doses of intravenous immunoglobulin. 7/7/09 Medical records received DOS 11/20/07 to 3/30/09. Assessment: Acute infective polyneuritis, Guillain-Barre syndrome. Patient presented with absent knee, achilles reflexes and absent superficial reflexes of feet. No rolling, no tactile sensation back. No weight bearing on legs, falls forward when sitting. Keeps legs extended, scissoring. Gradual improvement of neurological findings. Left side of gluteal fold 2 inch erythematous ring surounding pustule.". PARONYCHIA & CERUMEN IMPDevelopmental D/O-Hearing; ?'s 2 mos-screen passed; No crawl/pincer @ 9 mos 7/7/09 Medical records received DOS 11/20/07 to 3/30/09. Abscess finger.
