After vaccination, patient experienced the following side effects: oedema peripheral, pruritus, skin warm, , Received the shots on 10/23/2008 at 19:45 pm. After 24 hours developed itching and swelling of the forearm that extended to the entire arm. The are also became warm after 48 hrs. The swelling subsided after two weeks. She was only given Ibuprofen syrup. NoneShe is allergic to Amoxcillin but she outgrew it . Patient recovered.

After vaccination, patient experienced the following side effects: injection site pruritus, injection site swelling, paraesthesia oral, pruritus, Mother states child developed swelling to the left deltoid area approximately 8-12 hours after receiving flu vaccine. Also he complained of itching at the site and itching to his tongue. Mother states that both cold and hot compresses were applied as well as vicks vapor rub. Violet was applied to the tongue by mother. Child returned to Health Center after 24 hours was examined and instructed to use cool compresses only and to give 1 TSP Benadryl every 6 hours as needed.. NoneNone . During the same period patient was treated with None. Patient recovered.

After vaccination, patient experienced the following side effects: erythema, lymph node pain, oedema peripheral, pain in extremity, pruritus"Day after vaccine, left deltoid became red and swollen. Tender and warm to touch. Over the next several days signs and symptoms progressively worsened down arm to elbow area. On 1/14/09, client saw PMD, Dr. Brearly at Lexington Family Practice. Signs and symptoms continued through 1/15/09. C/O slight itching of arm, denies numbness. C/O lymph nodes in groin tender. Dr. Brearly prescribed prednisone 5 mg X 4 days, cipro 500 mg bid X 4 days, cool/warm compresses.". none reportednone . Patient recovered.

INFLUENZA (FLUZONE) side effect was reported on 03/01/2009. Male patient, child 5.0 years of age, was vaccinated with INFLUENZA (FLUZONE). After vaccination, patient experienced the following side effects: abdominal discomfort, abdominal distension, arthralgia, blood pressure decreased, c-reactive protein normal"By 11/28 evening upper left arm was red, swollen, itchy. Following day, more swelling, hot to touch, very itchy. 4 a.m. 12/14, severe swelling in knees, wrists, entire left arm huge and hot, hives over whole body, fever, itchy pain. ER, vomiting, dropping blood pressure, swelling and rash whole body. Dx serum sickness. Symptoms lasted 3-4 weeks. 2 days in hospital. Wheezing, swollen lips, trouble walking. A lot of joint pain. 3/20/09 Received PCP medical records of 12/3/08-3/2/2009. FINAL DX: serum sickness, amoxicillin vs vaccines Records reveal patient w/persistent cough & postnasal drip x 3-4 wks when seen 12/3 w/poor sleep secondary to cough. Initially had URI w/croup, phlegm now dx w/sinusitis & tx w/oral antibiotics. Exam on 12/12 was well child, had been seen in OT but stopped some time prior, being watched for social lag. RTC 12/16 s/p hospital admit visit for swelling of lips, knees & wrists. Tx w/nebs & antihistamines. Now had tongue tickling, sleepiness, urticarial rash all over, left arm very swollen, purplish, hot, swollen joints. Followup w/allergist. Allergy consult 1/9/09. FINAL DX: serum sickness like reaction likely penicillin allergy Suggested patient have FluMist in future. Continued to have wheezing & tx w/nebs. Abdominal distension/discomfort was likely due to constipation. Had trace proteinuria in hospital & UA repeated but not included. Was to return in 2 wks. 4/10/09 Received hospital medical records of 12/14-12/15/2008. FINAL DX: serum sickness Records reveal patient experineced pruritic erythematous maculopapular lesions and red swollen left arm, bilateral knee pain w/swelling x 2 days. Tx w/antihistamines, NSAIDs & nebs. Had orthostatic hypotension w/dizziness & nausea/vomiting in ER & tx w/IVF w/improvement. D/C to home w/PCP f/u. Clinic visit 12/29/2008 s/p hospitalization. FINAL DX: serum sickness like reaction, suspect amoxicillin reaction Doing well w/rare use of NSAID for joint pain, daily zyrtec & intermittent albuterol for coughing. Meds were". "Cough (lingering) ("sinusitis")""none PMH: recent sinusitis & on amoxicillin at admit. Hives, viral illness, bronchitis, OM, nosebleeds. Family hx: eczema, hypothyroidism, GERD, IBS.""Serum Sickness LABS:WBC 15.85(H), lymphs 22.0(L), neutros 72.4(H). ESR 5(N), CRP<0.5(N). Complement C3(N), C4 15.4(L). UA w/trace ketones/protein." . After vaccination, patient experienced the following side effects: immediate post-injection reaction, injection site erythema, local swelling, pruritus, throat irritation"Patient is well appearing and presented to clinic for varicella vaccine. Patient received first varicella vaccine too early (before one years old)and rececived a second varicella on 9-17-07. Patient also presented for flu vaccine. Patient has had flu shot in the past and denies any allergies to eggs or egg products. Patient received 0.5cc flu shot into Right deltoid without incidence. Patient received 0.5cc varicella SC into back left upper arm. Approximately 5 mins after injections patient complained of itchiness in left arm. Left arm injection site redden and had localized swelling approximately the size of a half dollar. Dr. was called to evaluate patient. Patient continued to complain of itchiness, but now spreading to throat and rest of body. Orders for benadryl 25 mg po. after 10 to 15 mis no change. Orders for EpiJr given. INFLUENZA (FLUZONE) side effect was reported on 03/25/2009. Female patient, 52.0 years of age, was vaccinated with INFLUENZA (FLUZONE). After vaccination, patient experienced the following side effects: back pain, blood test, borrelia burgdorferi serology, full blood count, hyperhidrosis"Itchy, hot rash broke out the night of the vaccination on elbows, moving the buttocks and then ankles. Feet sweating profusely. Lasted about 3 weeks. After another week, jaw, shoulders and back starting aching. Then right toes started tingling. Muscle twitches and spasms occurred primarily in back at first, now all over. The tingling progressed to both feet and some in the hands. MRI of brain and spinal cord showed nothing but age related degeneration. Blood tests all negative except ANA slightly elevated (133). Neurologist put me on prednisone and Gabapentin. Seemed to have little effect on pain and tingling. Tingling has progressed to groin, back and stomach at times. Pain persists in back, upper shoulders and neck. Tingling especially in toes and exacerbated upon exercise and hot bath. Neurologist just upped the dose of Gabapentin to see if it would alleviate the symptoms. Another blood test for ANA will be done.". "No, but getting over sinus infection"High blood pressure"MRI of thoracic, cervical and lumbar spine. MRI of brain. Blood tests for Lyme, thyroid, Vit D deficiency, and complete blood panel." . During the same period patient was treated with Lisinopril 5 mg Completed Bactrim for sinus infection two weeks prior. Patient recovered.

Child developed tactile temp 11-21-08 around 1pm followed by pruritus & hives on face, back, legs. Child was concurrently less active, more sleepy. T 100.7 at 19:41 at presentation to ED. Exam revealed wheals & decreased activity - nontoxic appearance. BENADRYL administered also TYLENOL.".
