Nationwide, at least 1.5 million people annually are harmed from drug errors, more than half of them in nursing homes, said the Institute of Medicine report, released yesterday.

After drug was administered, patient experienced the following problems/side effects: rash, toxic skin eruption. CONAN dosage: DAILY DOSE:20MG. During the same period patient was treated with ISOSORBIDE DINITRATE, PANALDINE, PLAVIX. Patient was hospitalized. Patient recovered.
