From simple mix-ups to fatal overdoses, medication mistakes happen, on average, once a day to every hospital patient, a new report says. Nationwide, at least 1.5 million people annually are harmed from drug errors, more than half of them in nursing homes.
Many Unfavorable Drug Studies Aren’t Published. Nearly a third of antidepressant drug studies are never published in the medical literature, and nearly all of those happen to show that the drug being tests failed to work.
Only Drugs That are Safe Should be put on the Market. Drug developers are just beginning to come to grips with recent FDA oversight reforms and how they will affect drug safety. But, it appears that industry and consumer advocates are not together on how well the overhaul will be able to prevent future drug safety scandals.

Consumer or non-health professional from ITALY reported LANSOX problem on June 21, 2007. Male patient, child 11 years of age, was diagnosed with dyspepsia, nausea and was treated with LANSOX. After drug was administered, patient experienced the following problems/side effects: visual disturbance. LANSOX dosage: 30 MG. During the same period patient was treated with RIFACOL. Patient recovered.

LANSOX problem was reported by a Consumer or non-health professional from ITALY on Aug 02, 2007. Female patient, 42 years of age, was diagnosed with haemorrhage, anaemia and was treated with LANSOX. After drug was administered, patient experienced the following problems/side effects: cardiac arrest, conjunctivitis, erythema multiforme, face oedema, mucosal inflammation, stevens-johnson syndrome, stomatitis, toxic epidermal necrolysis. LANSOX dosage: 30 MG (15 MG,2 IN 1 D) , ORAL. During the same period patient was treated with CARNITENE, CALCIJEX, EUTIROX, ALUMINUM HYDROXIDE AND MAGNESIUM TRISILICATE, NEBILOX, EPREX, RENAGEL, SUCRALFIN. Patient died.

Consumer or non-health professional from ITALY reported LANSOX problem on Aug 09, 2007. Female patient, 85 years of age, was diagnosed with abdominal pain upper, diabetic neuropathy, psychomotor hyperactivity and was treated with LANSOX. After drug was administered, patient experienced the following problems/side effects: renal failure acute, respiratory failure, rhabdomyolysis, somnolence. LANSOX dosage: 30 MG. During the same period patient was treated with GABAPENTIN, INSULIN LISPRO, RAMIPRIL, ASPIRIN, DILTIAZEM, SERENASE. Patient died.

Patient recovered.
