After drug was administered, patient experienced the following problems/side effects: cardio-respiratory arrest. METHOCARBAMOL dosage: unknown. During the same period patient was treated with ZOLPIDEM TARTRATE, OXYCODONE AND ACETAMINOPHEN, CITALOPRAM HYDROBROMIDE, OLANZAPINE, MODAFINIL, ANTIHISTAMINE. Patient died.
