Nationwide, at least 1.5 million people annually are harmed from drug errors, more than half of them in nursing homes, said the Institute of Medicine report, released yesterday.

TRAMADOL, PARACETAMOL, MORPHINE SULPHATE, SERESTA, FOSAMAX, LOVENOX. Patient was
