Male patient, 58 years of age, weighting 107.6 lb, was diagnosed with convulsion and was treated with PHENYTOIN. After drug was administered, patient experienced the following problems/side effects: confusional state, coordination abnormal, incorrect dose administered. PHENYTOIN dosage: 300MG HS PO. Patient was hospitalized. Patient recovered.

PHENYTOIN dosage: 400MG QHS PO. Patient was hospitalized. Patient recovered.

During the same period patient was treated with ATORVASTATIN CALCIUM, NIACIN, EZETIMIBE, ASPIRIN, FOSINOPRIL, PHENOBARBITAL. Patient recovered.

After drug was administered, patient experienced the following problems/side effects: coma, diabetic ketoacidosis, multi-organ failure. PHENYTOIN dosage: 150 MG;TWICE A DAY; ORAL. During the same period patient was treated with VALPROATE. Patient died.

During the same period patient was treated with VALPROATE, CEFTAZIDIME, FUROSEMIDE, FLUCONAZOLE, PANTOPRAZOLE, NADROPARIN. Patient was hospitalized. Patient recovered.
