Male patient , child 4 years of age, was vaccinated with DTAP (INFANRIX) on Dec 13, 2006. 12/12/06 mother stated patient went to emergency room and had infection in his left arm where he received vaccine. 12/13/06 mother called to say that MD said reaction was not from immunization but was an infection. Mother was asked to bring child to health department. Agreed to bring him 12/14/06. 12/14/06 Mother said MD said swelling going away and MD said was a reaction to immunization. Asked mother to sign release of information to obtain MMR recovery. Mother did not come to health department nor did she bring child. 12/15 mother said arm was fine.. . Attention deficit hyper disorder. Patient recovered.

"Mom stated on 1/30 child's left arm started showed marked swelling, and redness and was hot to the touch.The child ran a fever of 101. Mom took the child to her physician who stated it was an unusual vaccine reaction. He told her to give the cild Tylenol and Benadryl". . . Patient recovered.

"Patient received Dtap and IPV left deltoid and MMRV and flu Right deltoid. About 5 minutes after administering patient developed aheal on at front shoulder, deltoid, arm and itchy. No respiratory distress. Chest normal in exam. Patient fussy called about 3 hours later patient seen. Patient seen clinic 11/30/06 and hives resolved and no symptoms.". Oral med for anemia. NONE. DTAP (INFANRIX) problem was reported from KS on Feb 05, 2007. Male patient , child 4 years of age, was vaccinated with DTAP (INFANRIX) on Feb 05, 2007. "Around 7pm on 2/3/07 the pt was c/o pain in his left arm to his father. Father just thought he was c/o pain from having his shots the day before. On 2/4/07 at 4:30pm the mother noticed a problem with the pt' arm when bathing him. Mom noticed arm doubled in size, hard to touch with warmth and redness. She noticed a large bruise to left deltoid of 2-3cm. Mom called 24hr nurse hotline and was instructed to take to ER. At 5:30 pt was taken to ER. ER Dr. told mom it was a local rx with some ""damage to tissue"". Dr. instructed mom to observe arm for further problems. Dr. prescribed Keflex for 5days TID. Dr. instructed to apply ice prn and to make appt. with PMD in 2-3 days.". None. Zithromax. Patient recovered.
DTAP (INFANRIX) problem was reported from NV on Feb 05, 2007. Female patient , child 5 years of age, was vaccinated with DTAP (INFANRIX) on Feb 05, 2007. "On Thur., Jan. 31, 2007 the child received #5 dose of Dtap, #4 dose IPV, #1 dose of MMRV (had separate doses of MMR and Varicella at 1 year of age), #1 dose of Hepatitis A, and #4 dose of PCV7. The child took a 2 hour nap after receiving the immunizations. When she woke up she complained of pain in both legs. The morning of Thur., Feb. 1, the child developed a fever of 101 and continued to complain of leg pain. That same morning the family flew out of State. The Mother noticed that the child's right leg (immunization area) had a circular area of swelling about 4 inches in diameter with a center of redness about the size of a silver dollar. The left leg(immunization area) had an area of swelling and redness about the size of a quarter with a bruise in the center. Both sites were warm to the touch. Mother states that she treated her daughter with ice packs to the sites and administered Tylenol, Ibuprofen and Benadry until she could get an appointment with a doctor. The Mother was instructed by the doctor to continue her treatment and return if the child developed increased pain, redness, swelling or if any red streaks occur. The fever resolved after 2 days. On Sunday, Feb. 4, the leg pain, redness and swelling started getting better Today, Feb. 5. the leg pain has resloved. The swelling and redness has decreased in the right leg. The left leg the swelling and redness has resolved except for a bruise.". None. None Known. Patient recovered.

"Reported that patient's mom stated that he had redness, swelling, and moderate pain the day after the vaccines were given. He would not let his mother touch his leg; Motrin was given every 6 hrs and he soaked/played in a tub of warm water x 1; His temp was not taken; mom states was dancing this am. Instructed how to apply warm compresses and draw a line around the redness and swelling with a pen if it gets larger or has a red line going up the leg to come to the HD or take to her health provider.". . . "8/2/06 104 temperature within a week lesions noted to back, scalp, forehead, ears. Continued to develop new lesions daily. 9/19 Biopsy = Psoriasis. Severe psoriasis from head to toe developed with 1 month of vaccine. Numerous topical med given in addition to UVA light therapy 3 times each week. To date 2/2/07 continues to be severe.". NONE. "Patient received DTaP #4 and Prevnar #4 on January 2, 2007 and DTap #5 and Prevnar #5 on February 7, 2007. After vaccines, patient had fever over night of 101, pain at injection site, very mild edema. no erythema or echymosis". none. none. Patient recovered.
DTAP (INFANRIX) problem was reported from GU on Feb 12, 2007. Male patient , child 0.2 years of age, was vaccinated with DTAP (INFANRIX) on Dec 25, 2006. "15 minutes after administration of Dtap, Hib, Ipv, Prevnar note of red pinpoint blanching lesions on trunk that spread to arms and legs. No note of hives. No respiratory distress. Observation for >30 minutes done. No problems noted. Will do phone follow up tomorrow. Parents advised to watch out for hives/respiratory distress. Addendum: on 12/21/06 night time he cried for 3 hours continuously,". NONE. NONE. Patient recovered.

"5 month old male (s/p 2 doses of Rotateq), s/p surgery for intussuseption on 2/14/07. He presented with symptoms of vomiting, abdominal distention, absence of gas/bowel movement for 4 days, and questionable blood in the diaper 4 days ago. He is currently still in the hospital.". NONE. DTAP (INFANRIX) problem was reported from RI on Feb 19, 2007. Male patient , child 0.3 years of age, was vaccinated with DTAP (INFANRIX) on Feb 19, 2007. "Vaccinations adminstered Friday morning at approx. 10:30am. For the following 12 hours, patient experienced discomfort and moderately high temperatures (100 - 101 deg.). However throughout the weekend and into Monday morning, patient had crying fits for 1.5 - 2 hours at a time, high fevers, generally 101-102 deg (high of 103.5 at one point), and he began vomitting his bottle after each feeding. We have remianed in contact with a Nurse, as well as a physician from the pediatricians office.". . . Patient recovered.

"on 12/05/2006, the patient vomited 3 or 4 times and also had 3 episodes of diarrhea. 1 hour later, while in the office, he became lethargic, gray and needed stimulation and blow-by oxygen. He was transported by EMS to the emergency department.". Meds: Amoxil and Gentian Violet otic drops.. none. Patient was hospitalized Patient recovered.
DTAP (INFANRIX) problem was reported from MI on Feb 20, 2007. Female patient , 33 years of age, was vaccinated with DTAP (INFANRIX) on Feb 20, 2007. "Patient was sent to this office to receive diptheria, tetanus, pertussis immunization following recommendation of pediatrician after delivery. Medication was ordered from hospital pharmacy and administered. It was then noted that the medication received was ""Infanrix"" which is not approved for adults. CDC notifed, transferred to public health advisor who reports that patient should not experience any serious adverse effects from immunization. May have slight increase in local reaction if anything. Indicates that appropriate immunization would be Boostrix or Adacel.". "Advair diskus, Fluticasone SPR, Proair MDI". "Asthma, Migraine, Post-partum, allergic to IVP dye". Patient recovered.
