HIB (ACTHIB) problem was reported from PA on May 24, 2007. Male patient , child 1.0 years of age, was vaccinated with HIB (ACTHIB) on May 16, 2007. Information has been received from a licensed practical nurse concerning a 12 month old male with a history of foramen avale patent who on 21 Feb 2006 was vaccinated SC with a first dose of Varivax (lot# 652455/1018R). Concomitant vaccination included a fourth dose of ActHIB (lot # UE774AA) given IM. There was no illness at the time of vaccination. On 01 Mar 2006 the patient experienced vaccine site inflammation. He was seen for a doctor's visit and started on Omnicef 125/5 BID for 10 days. The patient's outcome was unknown. Additional information has been requested.. . foramen ovale patent. Patient recovered.

HIB (ACTHIB) problem was reported from PA on May 24, 2007. Female patient , child 1.1 years of age, was vaccinated with HIB (ACTHIB) on May 16, 2007. "Information has been received from an licensed practical nurse concerning a 12 month old female who on 19 Apr 2006 was vaccinated SC in the right upper thigh with a first dose of Varivax (lot # 652452/1016R). Concomitant vaccination on the same day included an IM Fourth dose in the left upper thigh of ActHIB (lot # UE788AA). On 27 Apr 2006, the patient developed vaccine site inflammation. She was started on Keflex (400/5mL 6cc TID x 7 days), warm compresses, and Motrin as needed for pain. No further information is expected.". . unknown. Patient recovered.

"Information has been received from a health professional concerning a 12 month old white male with gastroesophageal reflux disease, a history of premature birth at 34 weeks, and who was a twin, who on 27 Feb 2006 was vaccinated with a first subcutaneous dose of Varivax (Lot# 652455/1018R). Concomitant vaccine administered on the same day included a fourth intramusculuar dose of ActHIB (Lot #UE709AA). The patient was experiencing an upper respiratory infection at the time of vaccination and was being treated with over the counter medications. On 10 Mar 2006 the patient was seen in the office with a vaccine site inflammation. The patient was started on Ceftin, 125/5, 4 ml, twice per day for 10 days. It was reported that the patient recovered on 17 Mar 2006. The patient's twin sister experienced a vaccine site inflammation following vaccination with Varivax (WAES# 0605USA01887). "Vaccine 5/22/07 on 3/25/07 mom called to say fever 104 with hives and mon thought was a reaction to the HIB on 5/22/07 then was requested to come in for check to clinic (did not come in 2 hr to needs ride from friend). Then called MD on call with same sx, seen 9 days later no sx exam, no fever no hives.". None. . Patient recovered.

HIB (ACTHIB) problem was reported from MA on June 04, 2007. Female patient , child 01 years of age, was vaccinated with HIB (ACTHIB) on May 30, 2007. "Information has been received from a physician concerning an 11-month-old Asian female with no pre-existing allergies, birth defects, or medical conditions who on 16-MAY-2006 was vaccinated with a dose of MMR II. Concomitant therapy that day included a dose of ACTHIB. There were no other concomitant medications. On 23-MAY-2006 the patient developed full-blown measles. Subsequently, the patient recovered. There were no relevant diagnostic tests and laboratory data. There were no adverse events following prior vaccinations for the patient or her siblings. There was no product quality complaint. "Febrile seizure, on 6/9/07 lasting 3 to 5 minutes. No color change. rectal temp 103.9 degrees. Right TM distorted per doctor.". . eczema. Patient recovered.

HIB (ACTHIB) problem was reported from TX on July 24, 2007. Female patient , child 1.2 years of age, was vaccinated with HIB (ACTHIB) on July 02, 2007. Received vaccines below on 6/28/07. Later cried and did not want to bear weight on left leg. Taken to ER and diagnosed with left thigh immunization/decreased ambulation and prescribed Ibuprofen. Seen on F/U on 7/2/07 able to ambulate well. No fever. No swelling of left extremity.. None. none. Patient recovered.

HIB (ACTHIB) problem was reported from SC on July 26, 2007. Male patient , child 1.3 years of age, was vaccinated with HIB (ACTHIB) on July 26, 2007. "Had vaccine 7/17/07. Rash appeared 7/24/07 with fever. Highest temp. 105.3 R. Not eating well, fussy. Treating with Ibuprofen and Benadryl. Plenty of fluids. Seen in office 7/26/07 for RXN.". Amoxicillin x 10 days for ear inf - finished 7/24/07 per mom.. None. Patient recovered.

Patient returned home and later in the afternoon experienced a period of apnea which lasted about 1 1/2 minutes. Patient taken to hospital via ambulance and remains in hospital for observation.. None. None. Patient recovered.

HIB (ACTHIB) problem was reported from AZ on July 30, 2007. Male patient , child 1.3 years of age, was vaccinated with HIB (ACTHIB) on July 17, 2007. "Had mild fever 7-14-07. Not aware of fever on 7-15. Had seizure 7-15. Went to hospital ER diagnosed with ""febrile seizure"". Had normal C.B.C. and B.M.O. On 7-17, came to my office for follow up. Rash first noted then. Macular or flat spots especially on trunk, less on neck and extremities.". Motrin - 7-14 or 7-15.. None. Patient recovered.

HIB (ACTHIB) problem was reported from OH on Aug 15, 2007. Female patient , child 1.3 years of age, was vaccinated with HIB (ACTHIB) on Aug 14, 2007. 8-2-07 - was seen in office for rash on back only. Started on Keflex for folliculitis. 8-6-07 Rechecked in office since new lesions continued to appear - dx with chicken pox. Per triage note dated 6-15-07 mom dx with shingles.. Keflex. NKDA. Patient recovered.

HIB (ACTHIB) problem was reported from PA on Aug 21, 2007. Male patient , child 1.3 years of age, was vaccinated with HIB (ACTHIB) on Aug 15, 2007. Varicella vaccine breakthrough. Diagnosed with varicella on 8-14-07. Had T-100 for 2-3 days and approximately 20 vesicles with some crusting.. Albuterol Nebulizer and Pulmicort. Reactive airway disease. Patient recovered.

HIB (ACTHIB) problem was reported from AZ on Aug 21, 2007. Female patient , child 1.0 years of age, was vaccinated with HIB (ACTHIB) on Aug 21, 2007. "Vaccines administered Friday, 8-10. Fevers began Sunday evening on 8-12. Fevers have been inconsistent throughout the course of the weeks actually clearing for days and returning with a tendency to spike in the p.m. and remain low grade and unnoticed throughout the day. Fevers had been noted at 101.6 under the arm under Tylenol control. Diarrhea was inconsistently noted. Weight maintained from well-check to two week lapse. Appetite substantial with regression onto breast from usual table feedings. Monday, 8-13 small, defined, red spots began to be seen around mouth and by Tuesday evening, 8-14 the trunk had small defined red spots and progressed to lower extremities with fainter coloring. By Wednesday, 8-15 physician recommended treatment for hives with benadryl and claritin until clear. Noted rash clearing over next few days and discontinued meds on Friday, 8-17. Saturday, 8-18 she was noted by several acquiantances as having odd behavior and a full day without rest. (?possible seizure spell) Fever returned by Saturday afternoon and was controlled by Tylenol and Motrin. P.M. spiking was again noted. Sunday, 8-19 rash was seen returning on face. Monday morning, 8-20 within one hour the rash had been noted as spreading behind ears to entire trunk and throughout lower extremities. Again, the moderate-severe rash was small, defined red spots. Seen by physician and again diagnosed as possible hives. Physician did state viral infection more likely, but continue to treat for hives with Bendadryl and Claritin. As of today, Tuesday, 8-21 even with benadryl influence she is seen with a progressive severe covering of more confluent large raised rash groupings throughout face, neck, behind ears, trunk, genital area, and moderately throughout lower extremities.". . . Patient recovered.

HIB (ACTHIB) problem was reported from CA on Aug 22, 2007. Male patient , child 1.5 years of age, was vaccinated with HIB (ACTHIB) on Aug 20, 2007. About 12cm diameter region of blotchy erythema with induration (minimal induration at vaccine site only) - + itchy. . None. Patient recovered.

"Patient was mistakenly given MMRV and Varicella at the same time. Mother reported elevated temp and redness at the site of the MMRV. There aren't any adverse affects at this time, but we were told to complete this report anyway.". . None. Patient recovered.

"Patient with previous history of febrile seizures, got MMR as his on 8/13/07. Developed fever on 8/17/07 and had seizure exam 8/14/07 and examine 8/16/07. Patient found to have viral tracheoso (acute croup) after serous febrile seizure disease.". None. Febrile seizure March 07. Patient recovered.

HIB (ACTHIB) problem was reported from AR on Sept 05, 2007. Male patient , child 1.0 years of age, was vaccinated with HIB (ACTHIB) on July 13, 2007. "No fever. Child woke with ""spots"" on forehead (R) side and ant. neck and temple on 070707. More spots on 0712/07. Mother took child to doctor on 0712/07. TC from doctors office AM 0713/07 with above info. MD not sure if reaction from vaccines. TC to CDNS. Call mother to fill out VAERS and received more info. Mother state day 071307that spots have not increased. Denies any blisters> denies any fever. Mother states spots are barely raised.". Zyrtec 1/2 tsp po qd. "Seasonal allergies, fluid on brain - dx'd - no procedures yet - only CT Scans". Patient recovered.

HIB (ACTHIB) problem was reported from NJ on Sept 14, 2007. Female patient , child 1.0 years of age, was vaccinated with HIB (ACTHIB) on Sept 10, 2007. Fever over 101 for 3 days - fever broke and a fine macular rash on trunk seen. Dx as Roseola by doctor - parents felt the reaction was from immunization given on 8/24/07 Prevnar and Hib.. None. None. Patient recovered.

HIB (ACTHIB) problem was reported from MN on Sept 21, 2007. Female patient , child 2.0 years of age, was vaccinated with HIB (ACTHIB) on Sept 17, 2007. "Onset 9/12 6 days after shots given on 9/7/07 with fevers, irritability, dizziness and behavioral changes of quietness, nonplayful during the day. Fever, irritability and dizziness occurred even through night from 9/12/07 ----> 9/17/07. Two visit during this time were negative for bacterial infection. Fever lasted until 9/19/07.". None. None. Patient recovered.

HIB (ACTHIB) problem was reported from on Sept 24, 2007. Male patient , child 1.0 years of age, was vaccinated with HIB (ACTHIB) on Sept 21, 2007. "Information has been received from a health care professional concerning a study. An investigator reported that a 12 month old male on 23-AUG-2007 was vaccinated subcutaneously in the left arm with the first dose of ProQuad. Concomitant therapy included fourth dose (intramuscular, left thigh) ACTHIB and a fourth dose (intramuscular injection, right thigh) of PREVNAR. Study product was not administered. On 31-AUG-2007 the patient developed a fever and subsequently experienced a febrile seizure. The patient also experienced a rash (onset date not reported) that the reporting investigator felt was consistent with ProQuad. The reporting investigator felt that febrile seizure was related to study therapy. Treatment included TYLENOL for fever. Subsequently, the patient recovered from febrile seizure, fever and rash and continued in the study trial. Febrile seizure was considered to be an other important medical event. HIB (ACTHIB) problem was reported from IL on Oct 01, 2007. Female patient , child 1.1 years of age, was vaccinated with HIB (ACTHIB) on Sept 28, 2007. "Information has been received from a health professional concerning a 13 month old female, with no medical history and no allergies, who on 28-AUG-2007 was vaccinated with a first subcutaneous dose of ProQuad (Oka/Merck upgrade process) (Lot# 657782/0612U). Concomitant vaccines, administered on the same day, included a fourth intramuscular dose of Prevnar (Lot# B53981A) and a fourth intramuscular dose of ActHib (Lot# UF024AB). There was no illness at the time of vaccination and no adverse events following prior vaccinations. On 04-SEP-2007, at 6:00 a.m., the patient experienced a seizure, as stated by the patient's mother, and a high temp of 103.2, measured rectally. The patient was diagnosed with pneumonia of the lower right lung and was hospitalized for three days. Subsequently, the patient recovered. Additional information has been requested.". . None. "8/16/07 Hib #1 (VF086AA0, inconsolable within 30 minutes, lasted 14 hours, gave Tylenol at 11;30pm (at 12 1/2 hr) with resolution sx; no Tylenol prior. No prior or other vaccines given.". None. HIB (ACTHIB) problem was reported from MA on Oct 10, 2007. Male patient , child 1.3 years of age, was vaccinated with HIB (ACTHIB) on Oct 05, 2007. Approximately 3 wks after injection a red dot followed by a fluid filled vesicle appeared at site of injection. Parent advised to keep patient at home for 5-7 days or until vesicle dries and scabs over. Injection site location: upper (L) arm.. . N/A. Patient recovered.

Scattered hives and wheals - treated with oral steroid q. Doubt related to vaccine; suspect food allergy but reporting to be safe.. None. None. Patient recovered.

HIB (ACTHIB) problem was reported from FL on Oct 13, 2007. Female patient , child 1.3 years of age, was vaccinated with HIB (ACTHIB) on Oct 12, 2007. "The MMR shot was one of the environmental factors contributing to diagnosis of Autism. Other eczema, chronic constipation are other symptoms. 10/23/2007 MR received for WCC on 2/14/2003 to 3/29/2005. WCC on 2/14/03 with normal growth & dev however still walking with assistance at age 15 mon. Child had seen opthomologist for eval of stabismus/pseudostrabismus. 18 mon WCC 5/16/03 shows only saying 4 words otherwise doing well. Hearing eval 5/03 and myringotomy tubes placed 6/03. TC with MD 8/27/03 re: autism. MD advised to watch progression of speech and reassessment of skills now that pt is hearing well 2' to ear tubes. Speech delay noted at 2yr WCC. Speaks 3-5 words, no phrases. Receiving speech tx and early intervention. Multiple visits for ear issues-bleeding, drainage, OM. 3 yr check with developmental delay noted. DX: Autistic Disorder. PDD. HIB (ACTHIB) problem was reported from WA on Oct 15, 2007. Female patient , child 1.0 years of age, was vaccinated with HIB (ACTHIB) on Oct 10, 2007. Patient received the vaccine on 09/11/07 and on 09/22/07 a rash (tiny red bumps) showed up all over her body. It didn't bother her at all and it went way by 09/25/07. She had no other symptoms and we did not treat it in any way.. None. None. Patient recovered.

"According to my daughter, the mother of the child, he had an anaphylactic reaction that required resuscitation.
