INFLUENZA (FLUZONE) problem was reported from NY on Mar 14, 2007. Female patient , 19 years of age, was vaccinated with INFLUENZA (FLUZONE) on Dec 18, 2006. "Initial report received from a health care professional on 12 December 2006. A 19 year old, female patient (with a medical history of no known allergies) developed a flushed, red and itchy face, 2 days after she received Fluzone (lot number U2240AA) intramuscularly in the left deltoid. Menactra (lot number U2159AA) intramuscularly in the right deltoid and Gardasil (lot number unknown) on 20 November 2006. On 23 November 2006, the patient called to report red spots on hands. The patient did not have any illness at the time of vaccination. She was treated with Benadryl. She was not seen at the doctors office and has moved. It was reported that she recovered.". "Nevacor, Birth control pills, Celexa, Clonitin". NONE. Patient recovered.

"Demyelination - numbness and tingling from the breastbone down hips and pelvis into both legs all the way down to feet - pins and needles. Extreme fatigue, visual changes. Increase in migraine headaches - IV Prednisolone 3000 mg ---> home taper dose.". . . Patient was hospitalized Patient recovered.

Upper right arm-shoulder to elbow-red and warm to touch. Slight swelling upper right arm also.. On numerous meds for other health problems. "allergic to Sudafed, Doxycycline/COPD, Hypothyroidism, Hyperlipidemia". Patient recovered.

INFLUENZA (FLUZONE) problem was reported from CA on Mar 21, 2007. Female patient , 53 years of age, was vaccinated with INFLUENZA (FLUZONE) on Mar 16, 2007. "Initial report received on 17 January 2007 from the patient. A female patient (date of birth not reported), who has no concurrent illnesses or pre existing medical conditions, but who had tested positive for egg allergy (date of testing not reported), had received a right arm dose of Fluzone on 08 November 2006. The lot number, route and site of administration was not reported. Three days post-immunization, the patient had chills, numbness, tingling, and shooting pain in her arms that spread to all four extremities over the next month. At an unspecified amount of time later, the patient was evaluated by two neurologists at a hospital (contact information for the hospital was provided) and all tests were normal. Diagnostic testing included a Brain scan, and ""ANA"" test that were normal. The patient had not received any other vaccinations within four weeks of the vaccination on 08 November 2006. At the time of this report, the patient had not recovered. Follow-up information received on 20 February 2007. There was no additional information . Follow up information received on 15 March 2007 from the patient. The patient reported that she still had ""sharp pains in my bones"" and the numbness has continued now for four months. treatments received and diagnostic testings performed were not reported. Could not raise arm for 10 days and still not able to raise arm directly outward from side position without weakness or pain after more than 5 months. "Patient came to BOH on 3/27/07 to report adverse reaction from flu shot. She had no complaints until before Christmas (cannot give exact date) C/O burning in L arm, no redness, no swelling. Did not see MD. In Jan she felt lump in L arm (50cent size) MD said it was bursitis - now lump is 25 cent size, no pain, no redness.". . None. Patient recovered.

Child had removed the bandaid shortly after receiving the vaccination. Mother described that child has dry skin and alcohol used to cleanse area may have caused increased dryness. Presents to health department this AM with localized redness and induration of 4 cm. Circular area right deltoid with c/o pain. Able to move arm easily. I to use warm packs and Tylenol - return 3/30 if worse. School nurse also notified to check today and monitor.. None. None. Patient recovered.

Parents & child came over the clinic after 2 days of vaccinating stated child both thighs are sore and child was fussy. Noted both thighs are swollen with redness and hot to touch. No temp reported high. Told parents will have moderate soreness welling and redness and normal after having shots. No treatment done.. None. . Patient recovered.

Male patient , child 1.1 years of age, was vaccinated with INFLUENZA (FLUZONE) on Apr 02, 2007. 3/30/07 fever 103 rectal; continues as low grad as of 4/2/07. 3/31/07 light cough and runny nose; cough progressing. 4/1/07 rash - lower abd and back.. . None. Patient recovered.

INFLUENZA (FLUZONE) problem was reported from NY on Apr 05, 2007. Male patient , child 1.3 years of age, was vaccinated with INFLUENZA (FLUZONE) on Mar 30, 2007. Patient developed papular rash on lower and upper body and a temp of 101.9. Pt received Tylenol and a CBC was taken. Rash appeared 7 days after vaccination.. None. Down syndrome. Patient recovered.

Pt returned to facility two days after his vaccinations with hives on both legs. Mother stated it had started yesterday and was not spreading any more. No difficulty in breathing.. none. none. "9/28/06 shingles and Flu vaccine given separate arms. 10/30/06 wife called and stated 3-4 hrs after received vaccines pt didn't feel well, chills, teeth chattering, for 1/2 hrs, jumped out of chair, sweating, dripping water for 15 min. Bilateral shoulder and arm tingling 15 min wife thought from vaccines.". "Lopid, Dyazide, Toprol, Potassium, Fosamax, Rhinocort, Vit E, Calcium, Vit supp, Ultram prn". RASH AND ITCHING OVER FACE ARMS CHEST BACK. SEEN BY DR. 10ML OF BENADRYL GIVEN PO PER DR. DR. INSTRUCTED MOTHER TO GIVE ANOTHER DOSE IN 6 HRS. IF CHILD GET WORST TO GO TO THE HOSPITAL. Woke up at 4 AM crying, with fever 104 rectal, both legs red, swollen. Mom gave Motrin at 5am, cool bath done. 7:30AM, crying and seizures - ""child appeared scared - jerked, shaking for about 5 mins. Mom took to Dr at 9 AM.". None. . Patient recovered.

Child seen for initial refugee physical. Noted rt. knee pain on and off since 2006, diastolic heart murmur.X-ray rt. knee and cardio referral ordered. 3/23/07 translator called for mother stating child couln't walk due to pain in legs; sent to ER. X-rays and labs done, diagnosed with acute exacerbation of Rheumatic Fever. Dishcarged on Pen VF TID x10days, ASA 650 mg.q 6hrs x 5weeks, Bicillin IM q 4weeks. Follow up with cardiologist. 4/20/07 Symptoms are now resolved.". . records received 5/1/07-rheumatic fever. Patient recovered.

Loss of function of shoulder. Patient claims that injection caused permanent damage to shoulder joint.. . Osteoporosis. Patient recovered.

INFLUENZA (FLUZONE) problem was reported from NC on May 03, 2007. Female patient , child 1.0 years of age, was vaccinated with INFLUENZA (FLUZONE) on Apr 29, 2007. Patient received vaccines and later the next day broke out in a big rash that later became petechiae. Had several days hospitalization. Patient had severe swelling in legs and arms and was unable to walk for several days.. None. None. Patient recovered.

INFLUENZA (FLUZONE) problem was reported from OH on May 04, 2007. Male patient , 42 years of age, was vaccinated with INFLUENZA (FLUZONE) on Apr 27, 2007. "Initial report received from a health care professional on 02 November 2006. A 42 year old, male patient experienced pain and paraesthesia after he received an injection of Fluzone (lot number U2194AA) in the left deltoid (route and date of administration not reported). The pain was severe that he took Vicodin. The patient was referred to a neurologist. At the time of the report, the patient did not recover. Follow-up information received on 27 December 2006. Fluzone SV 2006-2007 was administered intramuscularly on 16 October 2006. Immediately post-vaccination, the patient experienced numbness and tingling of the chest wall and arm pain. The patient continues with pain. He had improved markedly and then symptoms reappeared. He was treated with therapy, a prednisone dose pack, and Darvocet. Unspecified x-rays were negative, and an EMG was negative. The patient was to follow-up with a neurologist. At the time of this report, the patient had not recovered. Follow-up information received on 20 April 2007. The patient had an EMG performed on 17 November 2006. The referring neurologist had the following impression: Musculoskeletal pain with a suggestion of possible very subtle brachial neuritis which was completely improved. The EMG nerve conduction study did not show any suggestion of neuritis when done. He has a very subtle left arm median neuropathy as seen in carpal tunnel syndrome. This would require further follow-up before any interventions. This case is upgraded to serious adverse event internally based on follow-up information.". . none. Patient recovered.

Patient received 11/1/06 started with rash 11/2/06 - admit to hosp 11/2-11/3 dx with hsp with arthritis. Later developed hematuria and proteinuria hsp nephritis with nephrotic syndrome. Required renal biopsy. Tx with Prednisone an then Cellcept. Pt also hypertensive requiring Isradipine and Lisinopril.. None. None. Patient was hospitalized Patient recovered.

"2/21/07-Mother called and stated that her daughter-(14 mths) received 3 vaccines on 2/6/07. She states her daughter had a reaction to vaccine and was told by her pediatrician to report reaction to health dept. First noticed 3 red dots on shoulder above vaccine site. Pt has been breaking out sporadically on face, neck and legs with chickenpox. Pt ran fever of 103F on 7th day after injection mother took child to doctor on 2/20/07 to examine rash.". None. None. Patient recovered.

INFLUENZA (FLUZONE) problem was reported from TX on May 07, 2007. Male patient , child 12 years of age, was vaccinated with INFLUENZA (FLUZONE) on Apr 30, 2007. "Hives, Cluster of 2 patients (patient identification provided) with generalized hives throughout the entire body, after receiving Adacel (lot number C2631AA); Fluzone (lot number unknown) and Menactra (lot number U1922AB); received from a health care professional in the USA on 24 April 2007. Case 1: A 12 year old, male patient (with a history of no known allergies) developed generalized hives throughout the entire body, 2 months after she received Adacel (lot number C2631AA), Fluzone (lot number unknown) and Menactra (lot number U1922AB) on 15 February 2007. He has the influenza vaccine annually. The patient denies any illness or medications at the time of the vaccinations. He was treated with Cortisone, Singular and Atarax. It was reported the hives resolved and the patient recovered. The second case ( the patient's sister) is detailed in Case 2007-01400.". . History of no know allergies. He has the influenza vaccine annually.. Patient recovered.

INFLUENZA (FLUZONE) problem was reported from TX on May 07, 2007. Female patient , child 11 years of age, was vaccinated with INFLUENZA (FLUZONE) on Apr 30, 2007. "Hives, cluster of 2 patients (patient identification provided) with generalized hives throughout the entire body, after receiving Adacel (lot number C2631AA); Fluzone (lot number unknown), and Menactra (lot number U1922AB), received from a health care professional in the USA on 24 April 2007. Case 1: An 11 year old, female patient ( with a history of no known allergies) developed generalized hives throughout the entire body, 1.1/2 months after she received Adacel (lot number C2631AA); Fluzone (lot number unknown), and Menactra (lot number U1922AB) and Gardasil (Merck, lot number 0014U ) on the 15 February 2007. She has the Fluzone vaccine annually. The patient denies any illness or medications at the time of the vaccinations. She was treated with Cortisone, Singulair and Atarax. INFLUENZA (FLUZONE) problem was reported from AZ on May 08, 2007. Female patient , 32 years of age, was vaccinated with INFLUENZA (FLUZONE) on Nov 07, 2006. "Numbness down (R) arm to hand. Felt ""woozy"" in the car; passed out after returning home, fell on floor; was unconscious for 2 hrs. Developed H/A, knot on her head. Took Advil, better next day.". "Amocicillin, Albuterol, MDl, Armour Thyroid". Hypothyroidism; Allergic to Cipro. Patient recovered.

"T/C from parent on 5/8/07 reporting rash all over infants body, head to feet. Denies fever, reports some GI vomiting of formula. Had child evaluated by Dr. who dx. rash as reaction to flu#2 vaccine received on 5/1/07. Parent recalls that a mild rash followed the first flu shot administered back in March, but at that time was much less and she thought it was probably a heat rash and did not relate rash to vaccines received on 3-20-07 (Flu#1, Dtap#3, IPV#3, Prevnar#3)". . .
