Complain of pain and swelling. Stated pain to arm started about 1 hour after shots. Noted Redness, swelling warm to touch and induration to left upper arm site where she received vaccines 3 days. "Nursing Comments: approximately 5 minutes after receiving 1st flu shot injection started to have a metallic taste in mouth, about 2 minutes later started to feel flushed, layed pt back on table and applied ice pack, about 2 minutes later, started to c/o dry mouth and that tongue felt like it was hanging on the back of her throat, received v/o to give pt 50 mg Benadryl po, given pt benadry at 1235 p.m at 12:40 pt complains of throat feeling full, states ""it feels funny"" pt complains of numbness around lips, Dr. listened to lungs and looked in throat, order received to given Decadron 4 mg, given Decadron 4mg IM right deltoid at 1245 states numbness around lips wearing off, pt coughing occasionally, speaking clearly, states ""I feel weird"" 12:57 Dr came in to assess and listen to lungs, instructions given to pt that is she has trouble breathing to go hospital right away, pt agreeable. pt d/c'd home at 1:02 p.m.". Plaquenil 200 mg QD Effexor XR 75 mg QD. Systemic Lupus Erythematosus. Patient recovered.

WOKE UP AT 11:30PM ENTIRE UPPER BODY WAS HURTING AND ACHING. HAD MODERATE TO SEVERE PAIN IN THE CHEST AND HURT WHEN TAKEN BREATH. THIS WENT ON FOR ABOUT2 HOURS. TOOK TYENOLAT 1:30A. EVERTHING CONTINUE TO HURT ALONG WITH UNCONTROLLABLE SHAKING. FELT BETTERAROUND 3:30A. NONE. NONE. Patient recovered.

On 12/5/05 pt called to report pain and tingling in left arm. Pt seen in clinic 12/7/05 and given rx for pain. 10/9/07 Received vax record from pcp which confirms lot #. 10/19/07 Reviewed neurology medical records of 4/11/07 which reveal patient experienced pain in left arm x 18 mos s/p flu shot FINAL DX: radial neuropathy.. Keppra; Neurontin; Hydrogamine; Darvon; Thyroid. "Seizure d/o; IBS; Hypothyroidism; Raynaud's; Osteoporosis PMH: nocturnal epilepsy, last seizure 7/06. RA, gynecomastia, cervical radiculopathy. Allergy: ASA, steoids, novocain, latex.". Patient recovered.

INFLUENZA (FLUZONE) problem was reported from CO on Oct 05, 2007. Female patient , 83 years of age, was vaccinated with INFLUENZA (FLUZONE) on Oct 05, 2007. "Patient received flu shot on 10/02/2007. Patient came to pharmacy today (10/05/2007) to show pharmacy intern her left upper arm where flu shot was administered. Patient's upper left arm was noted by pharmacy intern to be substantially bruised being purple and black in color. The bruising was noted to be circular and estimated to be about 3 inches in diameter and located towards the distal part of the deltoid muscle. Apparently the bruising has lessened, but was more extensive starting that night that she received the flu shot, extending from where it is now to all the way to where the flu shot was administered. Patient stated that her upper left arm was painful to move that night, but experienced no other symptoms. Today patient did not complain of any symptoms, nor pain associated with the current bruising. Patient stated that this never happened with previous flu shots. Patient stated that she is not on Coumadin, but is taking aspirin. Patient was advised that bruising is seen in some patients and that she should monitor the bruised area and seek medical attention if bruising gets worse or does not go away.". Aspirin. None known. Patient recovered.

"This patient received a Flu shot on 10/2/07 and started running fever on 10/5/07 of 104. She stayed home Friday and Saturday and had 104 fever on Saturday evening. On Sunday at 1620 with c/o fever and chills with generlized weakness and falls. WBC 7.0 At the time of this report the patient is an Inpatient at Piggott Community Hospital. 10/10/2007 MR received for hospital DOS10/07-09/2007 for the Assessment/DX of Fever, Weakness, Chronic A-fib. Pt presented to ER with c/o chills, generalized weakness, and 2 recent falls at home. Admitted for IVF and abx. PE significant for T'=102.7. Heart sounds irregularly irregular with 3/6 systolic murmur. 2+ edema in legs.". . "Heparin. PMH: Chronic A-fib, asthma, migraines, OA.". Patient was hospitalized Patient recovered.

"12 hours after the Flu Shot, pt complained that he could not move the arm of injection site, felt extreme discomfort (stiffness and pain)around the shoulder, neck and chest.He also had violent headache and nausea. He went to ER after four days of continuous discomfort and malaise, he was given a Vicodin rx for pain and sent home. His discomfort and malaise went away after one week.". "Received PPD L FA on 10-2-07 for yearly check. On 10-04, pt. came to me and showed me the PPD site. Induration was measured at 40mm and redness was also present. Pt. states that she noticed yesterday evening that it was red and swollen. No itching. Pt. was refered to occupational health on this same day. Chest X-ray was done and results were negative. Pt. states that it remained red and indurated until evening of 10-7-07.". None. None. Patient recovered.

"Pt received vaccination and stated felt dizzy, wanted to lay down; reclined on exam table and became diaphoretic, eyes rolled back, gray/ ashen color, did not respond to name or brisk shaking for approx 15-20 secs; weak/ tready pulse 70s, BP 84/40s, RR=12, Pulse ox= 99%; 911 called immediately; 0.3 mg SQ epi administered while awaiting ambulance; Pt responding by time ambulance arrived in approximately 5 minutes; BP 100s/70s, then 140s/70s, responsive, A&OX3; taken to St Luke's South Shore ER via ambulance by South Milw Fire Dept". . "Patient received fluzone on Tuesday, Oct 9. Called pharmacy on Wednesday complaining of swelling and pain at injection site. Advised to take tylenol and possibly use some ice for swellling. Phoned again Thursday and complained swelling and pain had become worse. Asked patient to come down for an assessment. Swelling present on shoulder. Also had pain on top of arm and under arm. Skin appeared spotty with red patches. No other areas of body involved. Patient feels fine except for pain in shoulder. Phoned family physician for patient, and requested that she be seen. Physician did not believe it was a serious reaction and that swelling would subside soon.". . . Patient recovered.

"I was given the vaccine & within 30min.I began having trouble breathing.I used my Xopenex inhaler & took 25mg Benadryl PO.I was also given Epi 0.3mg SQ via Epi-Pen.The reaction continued to progress & I was taken to the ED.There I was given another dose of Epi,2 more Xopenex resp.treatments,120 mg Sol-U-Medrol IV & placed on Bi-Papp.I was also give 60mg of Prednisone PO & another 60mg PO to be taken today.I was released from the Ed 5.5hr.later.Today I remain unable to speak above a whisper.ED doctor notifed my primary MD & I notifed my Pulmonary MD today.". . "Allergy to: Sulfa including sulfites & sulfonamides,Augmentin,Betadine/Iodine,All ""cillin"" drugs,Morphine. Also dx.with Migranes". Patient recovered.

INFLUENZA (FLUZONE) problem was reported from WA on Oct 12, 2007. Female patient , 52 years of age, was vaccinated with INFLUENZA (FLUZONE) on Oct 11, 2007. "Swelling at injection site - firm at the size of a baseball. Pain, welts, and low grade fever. Approximately 6-8 hours after injection. Prednisone 40 mg x 5d q 6 and Ranitidine 150mg BID, Benadryl 25-50 mg q 6, Hydromorphine 2mg PRN prescribed by ER doctor.". "VICODIN, DIAZEPAM, ZOLIPIDEM, GABAPENTIN, LEXAPRO, METHOCARBAMOL". . Patient recovered.
