PNEUMO (PNEUMOVAX) problem was reported from on Apr 03, 2007. Female patient , 63 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on Apr 02, 2007. "Information has been received from a registered nurse concerning a 63 year old female with rheumatoid arthritis, hypothyroidism, cataracts, rhinitis allergic, depression, anaemia and IVP dye allergy who on 16-MAR-2007 was vaccinated with Pneumovax (lot #655587/0882F), 0.5 ml, IM in the left deltoid. Concomitant therapy included CALTRATE, LEXAPRO, montelukast sodium, ADVAIR and ULTRAM. On 17-MAR-2007, the day after receiving the vaccination, the patient experienced symptoms of body aches, fever between 100-102F, flu-like symptoms, nausea, fatigue, and a sore left arm. The patient also ""felt faint"" and stayed in bed from 17-MAR-2007 through 20-MAR-2007. The patient sought unspecified medication attention. On an unspecified date, the patient had the following laboratory tests performed comprehensive metabolic panel (CMP), complete blood count (CBC) with differential, ""usual blood work"" (including iron, ferritin and TSH) results were not reported. Subsequently on an unspecified date, the patient recovered from body aches, fever between 100-102F, flu-like symptoms, nausea, fatigue, sore left arm and ""felt faint"". PNEUMO (PNEUMOVAX) problem was reported from NY on Apr 05, 2007. Male patient , 52 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on Apr 04, 2007. "Information has been received from a licensed practical nurse concerning a 52 year old male who on 13-MAR-2007 was vaccinated into the right deltoid muscle with a first dose of Pneumovax (Lot# 651406/0620F). Concomitant suspect therapy included tetanus toxoid, administered into the left deltoid. There was no illness at the time of vaccination. On 15-MAR-2007, the patient developed pain in the left deltoid at the injection site where the tetanus was administered. The nurse reported that the patient's pain radiated from the left deltoid up to the neck. The area was red and warm. It was also reported that the patient came to the clinic on 15-MAR-2007 complaining of a headache and stiff neck. The patient was reported as recovered on 15-MAR-2007. The nurse felt that the patient's experiences were important medical events. Additional information has been requested.". Unknown. None. Patient recovered.

"Patient received pneumonia vaccine in LUE on 3/26/07. Has had pain and redness over triceps area x 2 days. Denies numbness and tingling of LUE. No pruritis. Denies hx of previous AEFI. Reports receiving previous pneumonia vaccine since coming on in Oct 2005, but no entry on form. NKDA. Patient examined at bedside by doctor, (limited exam due to pt discomfort)- Erythema, increased warmth and tenderness extending from upper triceps area to just below posterior left elbow. Extremely TTP. No clavicular, cervical or axillary adenopathy. Neurovasc check of LUE intact. Contacted patient on 4/2/07. He said redness, warmth and tenderness surrounding vaccine site have all completely resolved. (This was all incidental to a hospitalization for left shoulder pain (possible left pectoralis tear) and bacteremia with infectious source likely dental procedure).". . Hx of facial and orbital reconstruction following MVA Tension Headaches.. Patient recovered.

PNEUMO (PNEUMOVAX) problem was reported from CA on Apr 11, 2007. Female patient , 62 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on Apr 10, 2007. "Information had been received from an office manager concerning a 62 year old female with no medical history or known drug allergies who on 03-APR-2007 was vaccinated intramuscularly into the left arm with a second 0.5 ml dose of Pneumovax (Lot # 651320/1008P). Concomitant therapy included CIPRO, atenolol, MICARDIS, SYNTHROID, testosterone gel, CENTRUM, cranberry tablet, ascorbic acid and baby aspirin. On 04-APR-2007 the patient developed swelling and redness along and underneath her left arm towards her elbow after vaccination. The patient visited an urgent care center and was given DECADRON and BENADRYL. It was noted that the patient had previously received a dose of pneumococcal 23v polysaccharide vaccine in 2000 and the patient did not experienced any adverse reaction at that time. At the time of report, the patient was recovering. A lot check was requested by the reporter. Swelling and redness along and underneath her left arm towards her elbow was considered to be disabling. "2-3 days after the 1st vaccine developed a deep unproductive cough with subsequent wheezing. Dr dx pneumonia and gave her Levaquin, Prednisone; 5 days later no better. 5 more days Levaquin with Albuterol. CXR was neg. Achiness missed 2 wks of work. Symptoms gone 4-9-07.". None. "Left arm where shot was given became very warm, swollen, experienced alot of pain and arm was very red. Upper part above the left breast had pain like when a person becomes upset. Had the shot on Friday 3/23/07 and did not go into work the following Monday, still experiencing pain and weakness, more pain at the elbow area, cannot lean it on the desk without the soft gel pad under my arm.". PNEUMO (PNEUMOVAX) problem was reported from on Apr 16, 2007. Female patient , 20 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on Apr 13, 2007. "Information has been received from a nurse practitioner concerning a 20 year old female with a prednisolone allergy, asthma and a history of pneumonia who on 13-NOV-2006 was vaccinated IM into the left arm with a dose of Pneumovax 23. Concomitant therapy included Lexapro. That evening at 19:00, the patient presented to the physician office and complained of achiness in her left arm. On 15-NOV-2007, the patient began to experience prodromal symptoms including: generalised body aches, fever of 102 F, low back pain, headaches, inability to extend her arm without pain, cellulitis and erythema of the left arm. The patient was treated with IV Rocephin. It was reported that they were not sure if the patient experienced an allergic reaction or cellulitis. On 15-NOV-2006, a CBC was performed and WBC was 20,000 and a chem panel was performed (results not provided). On 16-NOV-2006, a CBC and blood cultures were performed (results not provided), and a WBC of 15,0000. On 18-NOV-2006 the patient was seen with minimal erythema. On 20-NOV-2006, the patient recovered. No product quality complaint was involved. Achiness in arm, body aches, fever of 102 F, back pain, headaches, cellulitis, inability to extend arm and erythema of the left arm were considered to be other important medical events. "Fever, extreme persistent throbbing pain in shoulder and arm where the injuction was administered, chills and flulike sypmtoms, soaking night sweats, and severe headache (2 days in duration). The throbbing in the arm was so severe that sleep was not possible, this resolved in 18 hours, the arm remained moderately sore for an additional 2 days. General body weakness has persisted.". . Patient seen socially other day and tells me developed acute hear loss: c/o ENT ; 55% hear loss AD. In retrospect (but not documented in chart) he says hearing loss was abrupt and immediate.. "cozaar, prilosac, viagra, asa centrum silver". nka; aortic insuff; bell's palsy '91; gerd;htn; hx benigh positional vertigo. Patient recovered.

PNEUMO (PNEUMOVAX) problem was reported from CA on Apr 20, 2007. Male patient , 46 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on Apr 19, 2007. "Information has been received from a licensed practical nurse concerning a 46 year old male with type 1 diabetes mellitus, and no history of drug reactions/allergies. In 1994, the patient received first vaccination with Pneumovax 23. On 02-APR-2007, the patient was vaccinated with second dose of Pneumovax 23 (lot # 655152/0889F) 0.5 ml IM in the left deltoid. Concomitant therapy included ADACEL also given on 02-APR-2007 in the right arm. On 03-APR-2007, the patient experienced muscle aching and called the office. On 04-APR-2007, the patient was seen in the office with a fever of 100.4 degrees Fahrenheit (F). His left arm had a 8x8 area of induration. His fever had risen to 103 F with chills and sweating. He was placed on cephalahexin for 7 days. When he called the office on 06-APR-2007, his temperature had decreased to 98 F and the redness was decreased. On 10-APR-2007, at the time of reporting, he had recovered. The nurse reported that the muscle aching, fever, induration, chills, sweats, and redness were other Important Medical Events and they were disabling because the patient missed work. PNEUMO (PNEUMOVAX) problem was reported from ID on Apr 26, 2007. Male patient , 60 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on Apr 22, 2007. "I work at a mental health hospital, we are required to receive a PPD (T.B. Test) I was instructed to have this nurse (RN) that I work with on graveyard shift administer the PPD. However the nurse used Pneumovax 23 instead of the PPD serum and it was administered intradermally rather than IM or SC. In addition the injection site was hot cleaned or alcohol swiped. I contracted staff infection, within hours, and went to the hospital ER and was diagnosed with staff infection. I was put on 3 different antibiotics because my body kept reaction badly to the antibiotics. I was off work for 2 days. When I went back to work it was then that I learned from the nurse (Rn) that she had mistakenly used Pneumovax 23 instead of the PPD serum; she then asked if I had ever had this and I told her yes 4 months ago. She then said well your only suppose to get this vaccination once every 5 years or 3 year in some cases, but no in the time frame that I was dealing with. She then sent me to our local hospital. Where I was seen and prescribed a different antibiotic, However no labs were performed. But labs are to be performed this week 4-26-07. Since this event I contacted mfr to find out what I might be dealing with. Mfr sent me back all th info concerning Pneumovax 23. I was already experiencing joint swelling, itching, fatigue, and when I received the mfr info, regarding adverse reactions I felt somewhat received, at least there was a reason. I might add my employment has not assisted me in any way with this unfortunate incident. Unbelievable". . COPD. Patient recovered.

"3/17/07 pt called to report reaction to Pneumovax, developed pain, redness, and swelling at injection site, noted red streaks. Pt referred to ER as office closed on weekends.". "Synthroid, ASA, Voltaren, Nexium, Lipitor". "S/P CVA, hypothyroid, OA R hip, HX of melanoma, Sjorgren". Patient recovered.

PNEUMO (PNEUMOVAX) problem was reported from KS on May 02, 2007. Female patient , 31 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on May 02, 2007. "Patient's left arm became red, warm and swollen beginning the day of injection in the afternoon. One the second day she felt like the flu. Today she still has an area of erythema and tenderness of about 4cm by 5 cm distal the injection site without swelling, erythema or warmth.". "Lantus insulin, novopen, celexa, klonopin". Type 1 diabetes. Patient recovered.

PNEUMO (PNEUMOVAX) problem was reported from NM on May 03, 2007. Male patient , 68 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on Apr 19, 2007. Patient stated had spots on right deltoid 6 days after Tdap. He put hydrocortisone cream on it because it was slightly itchy. Two days later it developed erythema. It was not warm to the touch. Patient reports feeling normal otherwise.. HCTZ 25mg daily. "HTN, asthma". Patient recovered.

PNEUMO (PNEUMOVAX) problem was reported from TX on May 07, 2007. Female patient , 53 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on Apr 27, 2007. "Pt has redness on L arm (deltoid) 100m x 95m No induration present. Pt states this starts Sunday. First felt ""pain"" under Lt ear and Rt side of neck. Then that went away and she noticed the red Lt arm.". PPD. "NKDA, Diabetes Type II, Allergic rhinitis". Patient recovered.

PNEUMO (PNEUMOVAX) problem was reported from DC on May 07, 2007. Male patient , 43 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on May 07, 2007. "On 4/26 and 4/27 pt received a total of 5 vaccines (MGC, Hep A, Hep B, in left upper arm IM on 4/26) and Pneumococcal subq in left triceps area and Tdap IM in left upper arm on 4/27/07. Early on a.m. of 4/28/07, pt experienced fever, headache, body-aches, nausea and a reaction surrounding vaccine site in left triceps. Site was painful, hot to the touch, pruritic, red and edematous. There was a knot at the site the size of a goose egg and the redness, pain and swelling went from his left upper triceps area to his elbow. Systemic symptoms resolved over next 24 hours with round the clock administration of Tylenol. No adenopathy. No paresthesias in LUE. Pt has no history of previous vaccine adverse reactions. The reaction surrounding the vaccine site diminished significantly by 2 May. Fever workup was done and reveled no evidence of infection.". . Crohn's; Seasonal allergies; GERD. Patient recovered.
