PNEUMO (PNEUMOVAX) problem was reported from WA on May 07, 2007. Female patient , 85 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on May 07, 2007. "Pt presents at clinic 5/4/2007 with redness and warmth to right upper arm outer aspect, and c/o pain and tenderness since evening of 5/2/2007. Was assessed to be cellulitis by provider and treated with Keflex 500mg, one capsule by mouth three times per day for 10 days.". "acetominophen, aspirin, fosamax, HCTZ,Prozac". "no known drug allergies, type II diabetes, benign hypertension, osteoporosis, depressive disorder, mitral valve disorder, vericose veins, osteoarthrosis, debility". Patient recovered.

PNEUMO (PNEUMOVAX) problem was reported from PA on May 07, 2007. Male patient , 29 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on June 06, 2006. "Initial report received from a registered nurse at a health center on 21/Apr/2006. A 29-year-old male patient developed bouts of diaphoresis lasting a short time, a fever, heat and swelling at the injection site with pain in the right axillary node area the day after receiving the injection (route not reported) of TD, lot number U1703DA in the left deltoid and the intramuscular injection of Pneumococcal vaccine (manufacturer Merck), lot number L048P in the right deltoid on 19/Apr/2006. Per reporter the patient had no illness at the time of vaccination. The patient's symptoms began the day after vaccination, on the 20/Apr/2006 in the evening. On 21/Apr/2006 the patient's temperature was 98.6 degrees Fahrenheit. He was seen by a physician. No treatment was reported. The patient's recovery status was unknown at the time of this report. He is scheduled for a recheck on 24/Apr/2006. A new version was created, on 06/Jun/2006, to amend the product screen to Decavac. Decavac was the product administered, not TD ADS Adult as previously recorded.". "DIOVAN, LEXAPRO". The patient is on Diovan and Lexapro. He has a history of hypertension and anxiety. PNEUMO (PNEUMOVAX) problem was reported from VA on May 14, 2007. Female patient , child 11 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on May 14, 2007. "Severe redness, tenderness at the sight of the injection; diagnosed with cellulitis and osteomyelitis in the upper right arm which coincides with the sight of the injections; symptoms appeared less than 36 hours after the immunization; admitted to the hospital through the emergency room for treatment of the above conditions; bone scan and MRI done to confirm diagnosis; IV antibiotics for six weeks through PICC line after discharge from hospital. 05/17/2007 MR received for a 3 night admit (4/27-30/07) for cellulitis and osteomyelitis of the Right shoulder where she had received Pneumovax. She developed fever, malaise, and generalized weakness the day following her vax. Fever increased the next day to 103.1'F and erythema and induration of the right shoulder developed with decreased ROM. MRI revealed cellulitis and osteomyelitis of the right humerus so she was admitted for management. PE: WNL except severe erythema and induration of the R shoulder. Tender to touch. Decreased ROM. Txd with IV abx and discharged with PICC line for home IV abx. Final DX: Right Osteomyelitis and Cellulitis of the R proximal humerus. SIRS 2' to Cellulitis. Possibility of underlying immunodeficiency considering recurrent infections.". . none. Patient was hospitalized Patient recovered.

"Redness, swelling pain at injection site which extended down to elbow. Seen in ER 5/13 and then admitted 5/15/07 with diagnosis of cellulitis. Discharged 5/10/07 to home. 05/31/2007 MR received for a 1 day admission for cellulitis of the L upper arm at the site of her Pneumovax which was increasing, despite po abx. Pt/ c/o redness pain, and swellingx 2 days. Placed on po Zithromax, but redness and swelling were increasing. Txd with IV abx. Discharge DX: Cellulits after recent Pneumovax in the L arm. PNEUMO (PNEUMOVAX) problem was reported from CO on May 23, 2007. Female patient , 22 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on May 23, 2007. "Two hours after receiving Pneumovax, patient noticed pain around injection site. Pain gradually increased. 24 hrs later, she had 10x20 cm area of warmth, redness and tenderness. She could not abduct or posteriorly rotate shoulder due to 7/10 pain. C/o myalgias, no fever, no respiratory symptoms. Patient prescribed Allegra 180 mg po daily and ibuprofen for pain and inflammation; to apply ice pack and return if symptoms continued or worsened; to seek emergency medical assistance if any sign of systemic or respiratory symptoms.". "Zantac 150 mg BID, Lexapro 30 mg, levothyroid, Allegra 180 mg prn, Klonopin prn, Flonase 2 sprays each nostril daily, albuterol prn, multi-vitamins". PNEUMO (PNEUMOVAX) problem was reported from MN on May 24, 2007. Female patient , 63 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on May 23, 2007. "Information has been received from a physician concerning a 63 year old female patient with sulfonamide allergy who on 09-FEB-2007 was vaccinated intra-articular in her left shoulder joint with a dose of Pneumovax 23. On 09-FEB-2007 the patient experienced ""terrible pain"" that evening. on 12-FEB-2007 she was seen at the physician's office and was prescribed a steroid (unspecified) and pain reliever possibly Celebrex. She also went to the emergency room (ER) but was not admitted. She was given an injection of epivacaine and Kenalog. The physician said that the patient seemed to be doing better. The patient was recovering. The reporter considered the event to be disabling because the patient was unable to perform same physical activities such as gardening and kayaking. PNEUMO (PNEUMOVAX) problem was reported from NJ on May 24, 2007. Female patient , child 1.0 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on May 16, 2007. "Information has been received from a registered nurse (RN) concerning a 5 year old white female with no allergies and a history of ear tubes, who on 29 Jun 2001 was vaccinated subcutaneously in the left thigh with a first 0.5 ml dose of Variavax (lot # 637973/1884K). Concomitant vaccinations included a dose of Pneumovax. There was no illness at the time of vaccination. On 19 Mar 2006 the patient presented to the office with a spot like rash all over her body with complaints of itching. It was reported that the events began on 17 Mar 2006. It was diagnosed as a chickenpox breakout by the physician. No treatment was provided and no laboratory or diagnostic tests were performed. It was reported that the patient recovered. No product quality complaint was involved. Additional information is not expected.". . ear tube insertion. Patient recovered.

SHOT WAS GIVEN. PT REPORTED TO CLINIC THE FOLLOWING DAY WITH LOCAL REACTION TO SITE. MD WAS NOTIFIED, PT DID NOT HAVE SIGNS OR SYMPTOMS OF ANAPHYLAXIS. PT WAS ADVISED IF SYMPTOMS WORSENED TO REPORT TO ER FOR TREATMENT.". . . Patient recovered.

PNEUMO (PNEUMOVAX) problem was reported from CT on June 04, 2007. Male patient , 91 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on June 01, 2007. "Information has been received from a nurse concerning a 91 year old male with anaemia, prostatic nodule, hypertension and lactose intolerance who on 07-MAY-2007 was vaccinated intramuscularly in the left deltoid with Pneumovax 23 (Lot#654977/0888F). Concomitant therapy included finasteride, aspirin and lisinopril. Subsequently, the patient experienced that from his left elbow down to his left hand became grossly swollen. It was reported that the area ""was very hot, very erythematous, and very red"". The nurse reported that the reaction was described as a ""boggy elbow"" (mushy) and was the texture of the skin of an orange. It was also reported that it was difficult for the patient to raise his arm. The patient was treated with prednisone (dose not provided) immediately and then prednisone, 20mg daily for two days. The patient was also treated with Benadryl, dose not specified. The nurse reported that the reaction resolved in 3 to 5 days. PNEUMO (PNEUMOVAX) problem was reported from CT on June 04, 2007. Female patient , 72 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on June 01, 2007. "Information has been received from a registered nurse concerning a 72 year old female with asthma, osteoporosis and hypothyroidism and a history of kidney stone and migraine who on 12-FEB-2007 was vaccinated intramuscularly in the left deltoid with Pneumovax 23 (Lot#654977/08888F). Concomitant therapy included SYNTHROID, LIPITOR, aspirin, alendronate sodium, CITRACAL, PIORINAL, cholecalciferol, NOVO-GABAPENTIN and BENADRYL. On 13-FEB-2007 the patient experienced that her upper left arm became extremely red and bright in color, swollen, painful and hot. The nurse reported that the texture of the reaction was smooth. She also reported that the patient had limited use of her left arm because of the reaction. The patient was treated with BENADRYL (dose not specified) and prednisone (dose not specified). Subsequently, it was reported that the reaction had resolved and the patient had recovered within one week. The events were considered to be disabling by the reporter. Went to doctor and started on Prednisone 5mg and Cefadroxil 500mg (6 day & 10 day respectfully). All symptoms gone in 6 days.". . . Patient recovered.

"Information has been received from a healthcare worker concerning a 57 year old female secretary with a history of right lower lobe pneumonia (15-FEB-2007), rhinitis and fatigue and no known allergies who on 09-APR-2007 at 12:30 was vaccinated into the left arm with a first dose of Pneumovax 23 (Lot #655588/0883F) during follow up for her resolved pneumonia. There was no concomitant medication. On 10-APR-2007, the patient developed cellulitis from injection site to left elbow. The left elbow to upper arm was hot, red swollen and painful. There was no fever and distal pulse were 2+ to 3+. The patient was given LEVAQUIN 750 mg orally once a day for 10 days. The patient was to follow-up in 48 hours. On 25-APR-2007, the patient recovered. No product quality complaint was involved. Cellulitis from injection site to left elbow and left elbow to upper arm hot, red, swollen and painful were considered to be other important medical events. "Information has been received from a healthcare worker concerning a 65 year old female patient with allergies to cats and dogs and a lung nodule who on 17-APR-2007 at 10:30 was vaccinated into the left deltoid with a first dose of Pneumovax 23 (Lot # 655588/0883F). On 18-APR-2007, the patient developed cellulitis from injection site to elbow and the left shoulder to the elbow was red, hot and swollen. There was no fever but there was pain in the arm. On 19-APR-2007, the patient stopped in the physician's office and was given moxifloxacin hydrochloride (AVELOX) 400 mg once a day for 10 days. On 25-APR-2007, the patient recovered. Cellulitis from injection site to elbow, left shoulder red, hot and swollen and pain in arm were considered to be other important medical events.
