TDAP (ADACEL) problem was reported from OH on Apr 20, 2007. Male patient , 34 years of age, was vaccinated with TDAP (ADACEL) on Apr 20, 2007. "Headache, generalized muscle aches/pain, hot flashes, 7/10 left shoulder pain (injection site) and left are twitching.". . none. Patient recovered.

TDAP (ADACEL) problem was reported from MN on Apr 20, 2007. Female patient , 50 years of age, was vaccinated with TDAP (ADACEL) on Mar 16, 2007. "Tetanus reaction. Patient received her tetanus shot on March 8th. She initially just noted a little bit of redness and pain at the site but not too bad for several days. On the 12th, in the evening she noted that her third, fourth, and fifth digits on her right hand, the arm she got her tetanus shot in, were numb and tingly and upper arm and armpit, breast area was quite swollen and tender, She also noted a large red mark below the site of injection. She initially tried some Benadryl, ibuprofen 800 mg, cold compresses, etc. and was able to get some sleep that first night. By yesterday, however 800 mg of ibuprofen ongoing and regular Benadryl was not helping; in fact her pain and swelling is getting worse. She called out clinic and was recommended to come in for evaluation. Today, she feels things are little bit better although she now is noting some pain extending up into her neck and face and just a general funny feeling around her shoulder blade, her neck, and her face. She denies any fevers, chills. She denies any new skin changes. She denies any injuries to this area. She denies any symptoms in her lower extremities or her left arm and is otherwise feeling well. No nausea or vomiting. She had a good appetite, etc. No weakness in her right arm just pain. She has been able to continue to work. Allergies to Codeine and Morphine.". . Today 15 April 2007 patient has a headache rated on ""8"" on a scale of 1-10 that has been persisted since 0900 this AM and is increase in severity. Patient has been tearful since 1200.". TDAP (ADACEL) problem was reported from TX on Apr 26, 2007. Male patient , 52 years of age, was vaccinated with TDAP (ADACEL) on Apr 26, 2007. "Previous Td was March 2003. From HPI dated 12 Apr 07: Experienced onset of typical soreness at vaccination site followed by eruption of red raised areas. Has noticed associated dyesthesias with wearing his watch and clothing over his left arm to his wrist. Vague pain near posterior shoulder and neck. Reports previous hx of frozen shoulder after tetanus vaccine as a child. Left deltoid region with several raised erythematous and edematous plaques some with central papulovesicular change. No purpura, necrosis or crusts. Left hand no suspicious acral lesions. Apr 17: over past 5 days reports few new lesions on forearms and darker red color to older sites around shoulder. Also described episodes of 5/10 pain at shoulder site at rest and with arm elevation. Baseline 2/10 pain. No new lesions past day or so. Has several plaques of coalescing red to red brown papules over deltoid region with few scattered down to forearm. Some with focal superficial crusts. Diagnosis: 1. Brachial Neuritis 2.0 Arthus Reaction. Lesions, plaque 4/09/2007, lesion Vesicle 04/09/2007. Pain 04/09/2007 left shoulder and neck. Edema at injection site 30-50mm 04/07/2007. Paresthesia 04/09/2007 dysesthesia.". "allopurinol, pravastatin". Denies. Patient recovered.

TDAP (ADACEL) problem was reported from PA on Apr 27, 2007. Female patient , 53 years of age, was vaccinated with TDAP (ADACEL) on Apr 24, 2007. I developed neuritis and systemic polyneuropathy treated pain with Neurotin symptoms noted and 1 week after vaccination initially. 05/03/2007 MR received for multiple neuro consult OVs beginning 10/13/2006 to 3/27/2207 for c/o initially numbness of the lower extremities. Initial PE shows 2+ knee and 1+ ankle relexes and mild sensory disturbance over the ventral aspects of feet. Otherwise WNL. DX: Mild sensory neuropathy ideopathic in nature. Further F/U show sensory sx now increased and progressing to the arms. Numbness and tingling now to the mid thigh and left arm. Legs heavy. Now noted on 11/16/06 that sx. began after DPT vax. OV 12/11/06 with development of diffuse dysthetic pain in arms legs and trunk. DX: Ideopathic neuropathy.. Estrogen 1 mg daily. "NKA. PMH: Hyperlipidemia, Osteoarthritis. Hysterectomy. Allergies:NKDA.". Patient recovered.

TDAP (ADACEL) problem was reported from GA on Apr 27, 2007. Male patient , 16 years of age, was vaccinated with TDAP (ADACEL) on Apr 27, 2007. "2-3 minuites after receiving VZV and Tdap pt. began to feel faint and c/o dizziness and fainted.Skin cold and clammy, B/P 130/74. Pulse 64/reg. Pt.easily aroused in less than one minute after fainting. Alert and oriented to person, place,date,and situation.C/O generalized weakness and momentary numbness in lips and throat with no c/o shortness of breath..or difficulty breathing. After 10 min.of clinical observation: B/P 130/78, pulse 60/reg. while sitting. C/O soreness afterward in (L) shoulder. No swelling noted. Moves all extremities well. Left per wheel chair with god-grandmother enroute to Sumter Regional Urgent facility for evaluation. Child's father notified of incident and will be meeting child and god-grandmother at the facility. Pt. left in good spirits, no apparent distress and asked god-grandmother to follow-up with Health Dept. as to care rendered at the urgent care facility. "1 1/2 hours after vaccine administration the patient complained of a headache and lossed consciousness for 3 minutes. She regained consciousness, attempted to stand, and loss consciousness again for 3 minutes. Injection site painful and swollen. 05/23/07-records received C/O fever with chills. Temp in ED 99.4-BP 95/45. C/O pain over left deltoid area where he was given an injection. Some swelling present. Headache diffuse with no visual changes. No nausea or vomiting. Assessment:most likely allergic reaction to tetanus injection. Too soon for cellulitis. PE: Pain and tenderness over left deltoid area.. . "Allergy - Acetaminophen, Dx - GERD, Anxiety, Allergic Rhinitis, LBP records received 5/23/07-HX:whenever he has a temperature he has a similar type of headache. In the past he had a similar episode of fever last time when he received tetanu". Patient was hospitalized Patient recovered.

Client called health department on 042007 stating that she has had severe pain in left arm since receiving the immunization. Sometimes it keeps her up at night. Hurts when she rolls over on that arm. Still has achey feeling in that area.. Thyroid and diabetic pill daily. Diabetes and Thyroid. Patient recovered.

Celia states itch started that evening. Presents on 4/19 with red raised warm rash on left upper arm. There is 3 separate area 1 at injection site 2nd 2 below. Skin expected to return to normal. No fever and no aches. 6 days post Tdap vaccination she reports having 2 episodes of tachycardia (120-140) while at work and questions if related to Tdap vaccination. When evaluated Pulse was 96 and BP 110/84. Left deltoid was tender to touch and red and slightly swollen; limited ROM due to discomfort; afebrile. Obivious localized reaction from Tdap. TDAP (ADACEL) problem was reported from CO on May 02, 2007. Female patient , 32 years of age, was vaccinated with TDAP (ADACEL) on May 02, 2007. "At 2:00am on 4/26/07 patient woke up with a severe sore throat. She experienced chills and pain in lower extremities(legs). She did not take temp so unsure if she had fever. When she woke up later in am she felt like she had the flu. She experienced dizziness, headache and weakness in her legs. Called Dr. on 4/28/07 because she was not getting any better. Dr. could not see her that day so patient made an appointment for Mon 4/30/07. At that point in time, still had some weakness in her legs and patient still felt very bad. Dr. proceeded to do a strep culture on her. The strep culture came back negitive. Patient still complaining of slight headache as of today,but most of the other symptoms have subsided. Patients denies of ever having symptoms like this prior to a vaccination. Patient states that in 2003 she was given a Tetnus-Diptheria only, and the only side effect she had was a sore arm.". . Tdap vaccine recieved in left deltoid 3/29/07. Developed severe pain in right shoulder/arm 4/14/07, progressed to right shoulder weakness. Seen in Brigham and Women's Hospital Emergency Department on 4/16/07, diagnosed with acute brachial plexus neuritis. Treated with 1 gram methyprednisolone IV daily for 3 days. Patient had a previous similar episode of acute brachial plexus neuritis in left arm approximately 10 years previously which ocurred 2 weeks after an inluenza vaccine.". Aspirin 81 mg QD. None. Patient recovered.

"4/27 Itching, swelling & red at injection site; began earlier this week. No streaks or swelling of axillary through nodes. 4/29 Smptoms improved with Benadryl and cold compress, but still very itchy. 5/1 Continues to improve, redness greatly reduced". TDAP (ADACEL) problem was reported from VA on May 04, 2007. Female patient , 37 years of age, was vaccinated with TDAP (ADACEL) on May 04, 2007. "Client received Tdap (Adacel) on April 24, 2007. Her arm started to become sore that evening. Next morning, April 25, 2007, states, she ""could hardly move"". Client worked that day, and took Motrin 800 mg. TID. April 26, 2006, client states she felt the same way as she did on April 25. Her arm had begun to swell, was hot to touch, and she was very uncomfortable. States the swelling was ""pretty large and had a lump"" under the skin. Worked this day. Placed ice on the injection site that night. April 27, 2007 client states arem was still hurting. She took Motrin 800 mg. TID, applied ice to the are during the morning and evening, worked entire day. April 28 & 29, 2007, arm remained swollen but was starting to cool. She remained uncomfortable and was unable to sleep. April 27, 28, & 29, 2007 the shot site began to itch. April 30, 2007 client went to doctor. Arm was warm to touch but not hot, remained slightly swollen. Physician instructed client to apply ice to area QID. Was also given Dexamethasome 1.5 mg. titrated dosage (6 pills Monday, 3 pills Tuesday, 1 pill Wednesday, 1 pill Thursday). May 3, 2007 Client states she is feeling better, pain and swelling have resolved, she is applying ice to area PRN.". None. None. Patient recovered.

Patient also with palpitations last 2-3 days and throat irritation. . . Patient recovered.
