"Client called today to report that 2 days after receiving Zostavax vaccine she came down with shingles. They started on her right thigh, progressed to right hip and buttocks and down her right leg. The lesions are almost all scabbed over as of today. She did visit her doctor, but did not take any medications to treat the rash.". Prdnisone 5 mg daily. Rheumatoid arthritis. Patient recovered.

ZOSTER (ZOSTAVAX) problem was reported from GA on Oct 27, 2007. Female patient , 61 years of age, was vaccinated with ZOSTER (ZOSTAVAX) on Oct 25, 2007. "Information has been received from a physician concerning a female with no pertinent medical history or drug allergies who on approximately 01-AUG-2007 was vaccinated with a dose of Zostavax (Oka/Merck) in the deltoid. Concomitant drug therapy included cetrizine hydrochloride (ZYRTEC), ranitidine HCl (ZANTAC) and epinephrine (EPI-PEN). On approximately 30-SEP-2007 the patient experienced anaphylaxis characterized by chronic hives primarily on the extremities and swelling of the laryngeal area. The physician was consulted and the patient and was subsequently hospitalized for 4-5 days. Epinephrine was administered to treat the reaction. The patient was also treated with steroids for awhile. Lab diagnostic studies performed included full biochemistry labs at the hospital, electrolyte test revealing abnormalities, and a 63-64 value for lymphocyte count. The patient was noted to functional but not recovered. There was no product quality complaint. The physician considered hives and laryngeal swelling to be immediately life-threatening, disabling and an other important medical event. Additional information has been requested.". "Friday, October 26th, 2007 = Intense stomach pain. Monday, October 29th, 2007 = Continued pain and prescence of a rash. Went to emergency room and had tests listed below. Was shown to have blood in the urine and given Bactrim DS and Vicodin. Tuesday, October 30th, 2007 went to primary care physician and was diagnosed with shingles. Given Gabapentin and Famvir as treatment. As of today's date, Thursday, November 1st, 2007, patient is experiencing less pain and rash is ""less blotchy"". Patient says symptoms ""come and go"" and she just paces herself with day to day activities.". "Carbidopa/Levodopa, Lisinopril, Effexor XR, and Hydrochlorothiazide. Then on 24-SEP-2007 the patient was treated with levofloxacin (LEVAQUIN) x 7 days as well as a dose of fluconazole (DIFLUCAN) for an unspecified reason. The patient was also treated with famciclovir (FAMVIR) x 7 days. On 01-OCT-2007 the patient presented to the office with complaints of a painful rash in the right flank area (at approximately T10-T12 dermatome) consistent with herpes zoster that began on 29-SEP-2007. Shingles vaccine: Red raised area larger than a silver dollar with itching occurred the next day after shot. Returned to clinic three days after shot. Was given five days of Cephalexin 500 mg twice a day. Six days after shot the redness is starting to recede and itching is decreasing. I had shingles when I was 13.. "Information has been received from a physician concerning a 60 year old female with stomach virus, allergy to Levaquin and drug hypersensitivity to codeine who on 10-OCT-2007 was vaccinated with a dose of Zostavax (Oka/Merck) (invalid lot# 0761409). There was no concomitant medication. In October 2007, the physician reported that ""after receiving Zostavax (Oka/Merck) the patient had severe motor weakness of all extremities, but with no sensory loss, onset reported as 26-Oct-2007. The patient then saw a neurologist then admitted the patient immediately to the hospital. At the hospital the patient was diagnosed with Guillain-Barre Syndrome and was hospitalized for 6 days. Unspecified medical attention was sought. No product quality complaint. The physician reported that the patient is recovering and is going through physical therapy"" Guillain-Barre Syndrome was considered serious due to hospitalization, immediately life-threatening and disabling. Upon internal review, Guillain-Barre Syndrome was also considered an Other Important Medical Event. Additional information has been requested. 11/19/07-records received for DOS 10/30-11/4/07-DC DX: Guillain-Barre syndrome. Benign essential hypertension, severe cerebromicrovascular disease, headache and low grade fever related to intravenous gamma globulin (presumed). Presented with several day history of pure ascending paralysis approximately two weeks after they vacationed in which she had a multitude of mosquito bites. Treated with IV gamma globulin.". . "Information has been received from a licensed practical nurse (LPN) concerning a retired 77 year old female with asthma, aortic stenosis, paroxysmal atrial fibrillation, left ventricular hypertrophy, coronary artery disease, and elevated cholesterol, who was vaccinated with Zostavax (Oka/Merck) (lot#""6743U""). The patient developed a mild case of shingles after having been vaccination with Zostavax (Oka/Merck). The patient was seen in the office and ordered valacyclovir hydrochloride (VALTRIX). Additional information received from a nurse practitioner reported that the patient developed a rash on 04-SEP-2007, and was seen on 07-SEP-2007 for shingles. Treatment included famiciclovir (FAMVIR) and deltasone (PREDNISONE). It was reported that the patient had recovered. No product quality complaint was reported. No further information was available.
