Female patient, child 7.0 years of age, was vaccinated with INFLUENZA (H1N1) (H1N1 (MONOVALENT) (UNKNOWN)). Patient symptoms: Injection site swelling, "The exact time of the shot is a guesstimate. She has had local reactions to each of the flu shots since the first flu shot she was given. Trying to eliminate what it could be, we requested a preservative free vaccine for her second H1N1 dose. She complained immediately after the shot was given that it was burning. Thirty minutes after the shot her deltoid muscle swelled where she had received her vaccine. She complained of not feeling well but was unable to be more specific. An hour and a half after receiving the vaccine she had a rash that extended above the site as well as down to the wrist. she became pale and would only communicate via head nods.I gave her 2 childrens benadryl and watched her. Forty five minutes after being given the Benadryl she began to perk up and the rash didn't spread any further at that time. She was given 1 Benadryl about an hour prior to the shot to keep her from reacting. The next morning she had a 2.5~ X1.5~ red wheal in the area of her shot. The rash was still present but was only localized to the arm that was vaccinated. She also had a severe eczema flare 24 hours later following her shot that was all over her body. By Friday of the same week her eczema flare had calmed down and her wheal was only faintly visible." Was not sick at the time of the vaccine."She has documented environmental allergies as well as a penicillin allergy. She is currently taken allergies for the environmental allergies. She also has eczema flares with certain foods, (egg, milk, potatoes)." . Patient recovered.

It became swollen, started burning/itching and there was heat in the red area. The swelling and redness progressed to the elbow. There was pain in the elbow joint once the swelling and redness hit there. I was treated at an urgent care facility, was given a shot of antibiotics and then a prescription for a sulfa antibiotic. On the 31st I went to emergency room because the swelling and redness was not abating. I was given Augmentin. Both emergency room and the urgent care facility said it was MRSA. But there were no open wounds. No blood work was done and no one reported the reaction." none"COPD, emphysema, which is why the Pulmonary doctor sent me for the shot. Patient symptoms: Injection site swelling, "Information has been received from a registered pharmacist concerning a 66 year old female patient with chronic obstructive pulmonary disease, sulfa, iodine and codeine reaction/allergy who on 19-NOV-2009 was intramuscularly vaccinated with a 0.5 mL dose of PNEUMOVAX 23 (Lot#662661/0978X). Concomitant therapy included H1N1. The pharmacist reported that after received the PNEUMOVAX 23, on 21-NOV-2009, the patient experienced red and swollen at the injection site. No lab tests were performed. The patient called to the pharmacist for medical attention. ~3-4 days after the start of the reaction~, the patient had recovered. Follow up information has been received from the pharmacist that reported that the patient did not need to saw her doctor and that the symptoms resolved. No further information is expected." Patient symptoms: Injection site swelling, "Information has been received from a healthcare worker and from Claim Management Services, INC. (claim number 30100122931-0001) concerning a 58 year old female who on 10-JAN-2010 was vaccinated with PNEUMOVAX 23 in her left arm (dose and route not reported). Concomitant therapy included H1N1. The patient stated that on 11-JAN-2010 she had an infection in left arm from PNEUMOVAX 23 shot. The patient experienced swelling, redness, warmth and pain in her arm. Unspecified medical attention was sought. At the time of reporting, the patient had not recovered from the event. Follow-up information was received from the patient via Claim Management Services. The patient stated that there was not anything wrong with PNEUMOVAX 23. The patient's arm became infected and swollen after vaccine shot. The pharmacist explained that her dose had come from a multi-dose vial and the vial was open. This contamination led to the patient's infection. Whether an open vial or an unopened vial was used, contamination can occur with preparation. The patient had already been examined in the emergency room and after examination by the doctor the patient was found to have cellulitis. She was given an antibiotic and a pain medication LEVAQUIN and VICODIN to treat her symptoms. The patient had eight hours of sick time. Additional information has been requested." UnknownUnknown . Patient symptoms: Injection site swelling, "I changed my son into pajamas and at that time noticed his upper arm had swollen and turned red around the injection site. There is approximately a 4 inch by 3 inch round area very swollen, red, raised, and firm. No white discoloration is showing in his arm (it doesn't look like a hive). The swelling is obviously related to the injection as the injection puncture site is in the exact middle of the swollen, red, area. His upper left arm is disproportionately larger than the other arm. I gave patient 5mL Hydroxyzine syrup at approximately 1:45 AM in case it was an allergic reaction. It is approximately 50 minutes later and there is no sign of improvement." "Mild, occasional cough with congestion (the only symptom before shot). Had cough for about 2 weeks. The NP thought it was either""Allergies: Eggs, Milk, Wheat, Fish Illness-Induced Asthma I am aware that the flu shot is made with egg. My son received the flu shot every year and had no problem, even after he was allergic to eggs. He received 3 flu shots last year (2 swine flu shots and 1 seasonal flu shot) and had no reaction." .
